摘要
目的分析2型糖尿病对ICU机械通气呼吸机相关性肺炎(VAP)患者病情变化及生存情况的影响。方法收集2017年3月-2019年3月在我院ICU行机械通气的89例患有VAP的患者作为研究对象,按照有无合并2型糖尿病将患者分为合并糖尿病组与无糖尿病组。其中合并糖尿病组45例,无糖尿病组44例。两组患者均在ICU病房行机械通气治疗,日常监测血糖变化并行基础治疗。对比两组患者的昏迷及健康状况,记录并比较气管插管时间、ICU住院时间及MV持续时间,观察不良事件(心血管疾病、肾功能衰竭、神经系统病变)及死亡情况。结果合并糖尿病组患者的气管插管时间、ICU住院时间、MV持续时间、不良事件总发生率、急性生理学与慢性健康状况(APACHE-Ⅱ)评分高于无糖尿病组(χ^(2)=4.229,P<0.05);合并糖尿病组的格拉斯哥昏迷量表(GCS)评分低于无糖尿病组(χ^(2)=5.167,P<0.05)。结论 2型糖尿病会加重ICU机械通气VAP患者的病情进展,增加不良事件的发生,导致预后效果差。
Objective To analyze the effect of type 2 diabetes on the changes and survival of ICU patients with mechanical ventilation and ventilator-associated pneumonia(VAP). Methods A total of 89 patients with VAP who underwent mechanical ventilation in our hospital from March to May 20, 2017 were enrolled. Patients with or without type 2 diabetes were divided into diabetic group and non-diabetic group. There were 45 patients with diabetes mellitus and 44 patients without diabetes. Both groups were treated with mechanical ventilation in the ICU ward, and daily monitoring of blood glucose changes was performed in parallel with basic treatment. The coma and health status of the two groups were compared. The time of tracheal intubation, ICU stay and MV duration were recorded and compared, and adverse events(retinopathy, cardiovascular disease, renal failure, neurological disease) and death were observed.Results The tracheal intubation time, ICU stay, MV duration, total incidence of adverse events, acute physiology and chronic health status(APACHE-II) scores in the diabetic group were higher than those in the non-diabetic group(χ^(2)=4.229,P<0.05);The Glasgow Coma Scale(GCS) score was lower in the group than in the non-diabetic group(χ^(2)=5.167,P<0.05). Conclusion Type 2 diabetes can aggravate the progression of ICU mechanical ventilation VAP patients and increase the incidence of adverse events, resulting in poor prognosis.
