摘要
目的研究并探讨血液检验红细胞参数在贫血鉴别诊断中的检验价值。方法100例贫血患者,依照贫血类型不同分为缺铁性贫血组和地中海贫血组,每组50例。另选择同时间段50例体检健康者作为对照组。三组均进行血液检验。比较三组红细胞参数[血红蛋白(Hb)、红细胞计数(RBC)、平均红细胞血红蛋白含量(MCH)、平均红细胞体积(MCV)、红细胞分布宽度(RDW)]。结果对照组Hb、MCH、MCV、RBC高于缺铁性贫血组,RDW低于缺铁性贫血组,Hb、MCH、MCV高于地中海贫血组,RBC、RDW低于地中海贫血组,差异均具有统计学意义(P<0.05)。地中海贫血组的Hb(100.31±13.02)g/L、RBC(5.09±0.43)×10^(12)/L、MCH(24.06±3.74)pg高于缺铁性贫血组的(84.98±11.07)g/L、(3.16±0.39)×10^(12)/L、(19.57±2.64)pg,MCV(70.11±6.97)fl、RDW(17.05±3.12)%明显低于缺铁性贫血组的(75.12±7.13)fl、(22.10±4.18)%,差异具有统计学意义(P<0.05)。结论通过血液检验贫血患者的红细胞参数,有助于鉴别缺铁性贫血与地中海贫血的类型,提高诊断准确性。
Objective To study and discuss the value of red blood cell parameters in the differential diagnosis of anemia.Methods A total of 100 anemia patients were divided into iron deficiency anemia group and thalassemia group according to different types of anemia,with 50 cases in each group.Another 50 healthy subjects were selected as the control group.Blood tests were performed in three groups.The red blood cell parameters[hemoglobin(Hb),red blood cell count(RBC),mean corpuscular hemoglobin(MCH),mean corpuscular volume(MCV),and red cell distribution width(RDW)]of the three groups were compared.Results The Hb,MCH,MCV and RBC of the control group were higher than those of iron deficiency anemia group,RDW was lower than iron deficiency anemia group;the Hb,MCH and MCV of the control group were higher than those of thalassemia group,and RBC and RDW were lower than those of thalassemia group.All the difference was statistically significant(P<0.05).The Hb(100.31±13.02)g/L,RBC(5.09±0.43)×10^(12)/L and MCH(24.06±3.74)pg of thalassemia group were higher than(84.98±11.07)g/L,(3.16±0.39)×10^(12)/L and(19.57±2.64)pg of iron deficiency anemia group,and MCV(70.11±6.97)fl and(17.05±3.12)%were obviously lower than(75.12±7.13)fl and(22.10±4.18)%of iron deficiency anemia group,and the difference was statistically significant(P<0.05).Conclusion It is helpful to distinguish iron deficiency anemia and thalassemia and improve the diagnostic accuracy by blood test of red blood cell parameters of anemia patients.
