摘要
目的分析肿瘤专科医院胸外科非计划再次手术患者的临床资料,总结非计划再次手术的原因、特点及经验教训,为提高胸外科的手术操作水平及围手术期的医疗服务质量、保障医疗质量安全、降低非计划再次手术的发生率提供客观依据。方法回顾性分析2012年9月至2020年6月我院胸外二科的47例非计划再次手术患者的临床资料。分析造成非计划再次手术的原因及两次手术的时间间隔。结果 47例患者行非计划再次手术,其中以恶性肿瘤居多,占83.0%(39/47)。再次手术的原因主要是术后再出血(多在术后5 d内发生)和乳糜胸。结论临床医师首先应当增强责任心,其次要不断提高手术水平,加强围手术期的管理,对已发生的非计划再次手术的原因进行分析并不断总结经验,提出预防措施,尽量降低非计划再次手术的发生率,不断提高胸外科整体诊疗水平。
Objective To analyze the clinical data of patients undergoing unplanned reoperation in thoracic surgery department of specialized cancer hospital,and summarize the reasons and characteristics of the planned reoperation and the experience and lessons,in order to improve the operation level of thoracic surgery department and the quality of perioperative health services,ensure the safety of medical quality,and reduce the incidence of unscheduled reoperation provides objective basis.Methods The clinical data of 47 patients who had unplanned reoperation from September 2012 to June 2020 in our hospital were analyzed retrospectively.The reasons for unplanned reoperation and the time interval between the two operations were analyzed.Results Forty-seven patients underwent unplanned reoperation,most of which were malignant tumors,accounting for 83.0%(39/47).The main causes of reoperation were postoperative rebleeding(most of them occurred within 5 d after operation)and chylothorax.Conclusion Clinicians should first enhance sense of responsibility,then continuously improve the level of surgery,strengthen perioperative management,analyze the causes of unplanned reoperations that have occurred,constantly summarize experience,and propose preventive measures,in order to reduce the incidence of unplanned reoperation,and constantly improve the overall level of diagnosis and treatment of thoracic surgery.
