摘要
目的探究分析癫痫缓解期患儿减停药后2年内病情复发情况,并对诱导复发的相关因素进行分析。方法随访2016年3月至2018年3月在本院经抗药物治疗后处于癫痫缓解期的患儿168例。回顾性分析患儿的临床资料,分析引起癫痫复发的相关因素。结果随访2年,168例患儿中40例复发,复发率为23.81%。年龄<6岁、治疗前病程≥1年、治疗前癫痫持续、颅内有病灶、抗癫痫药物治疗周期≥1年、减停药前有痫性放电、减停药时间<6个月以及减停药前EEG表现异常均为癫痫患儿减停药后复发的危险因素。结论临床中癫痫患儿减停药应注重个体化原则,针对高危因素采取具有针对性的干预措施有助于降低癫痫缓解期患儿减停药后的复发率。
Objective To explore and analyze the disease recurrence in children during epilepsy remission within 2 years after drug reduction and withdrawal,and to analyze the related factors that induce recurrence.Methods A total of 168 children during epilepsy remission after anti-drug treatment in our hospital from March 2016 to March 2018 were followed up.Their clinical data were retrospectively analyzed.The related factors that caused epilepsy recurrence were analyzed.Results In the 2 years of follow-up,there were 40 recurrence cases in the 168 children,with recurrence rate being 23.81%.The following factors,including age younger than 6 years old,course of disease no shorter than 1 year before treatment,persistent epilepsy before treatment,intracranial lesions,AEDs treatment cycle no shorter than 1 year,epileptic discharge before drug reduction and withdrawal,drug reduction and withdrawal shorter than 6 months and abnormal EEG manifestations before drug reduction and withdrawal,were risk factors of recurrence.Conclusion Clinically,drug reduction and withdrawal should follow the principle of individualization in epilepsy children.Taking targeted intervention measures for high-risk factors helps to reduce the recurrence rate after drug reduction and withdrawal in children during remission.
