摘要
目的探讨基于跨理论模型的健康教育在慢性阻塞性肺疾病(COPD)患者肺康复随访管理中的应用效果。方法选择2019年2月~2020年1月佛山市南海区第九人民医院住院治疗后病情好转拟出院的80例COPD患者作为研究对象。根据随机数字表法分为观察组和对照组,每组各40例。对照组实施常规的健康教育,观察组实施基于跨理论模型的健康教育。比较两组干预前及干预3、6个月后的疾病自我管理水平、肺功能及生存质量。结果观察组患者在干预3、6个月后的自我管理水平和生活质量各维度得分、第1秒用力呼气容积(FEV_(1))、FEV_(1)/用力肺活量(FEV_(1)/FVC)及FEV_(1)占预计值的百分比(FEV_(1)%pred)均高于干预前,且高于对照组,差异有统计学意义(P<0.05)。结论基于跨理论模型的健康教育能有效改善COPD患者的疾病自我管理能力,促进肺功能康复,提升生存质量。
Objective To explore the application effect of health education based on transtheoretical model in pulmonary rehabilitation follow-up management of patients with chronic obstructive pulmonary disease(COPD).Methods A total of 80 COPD patients who were admitted to the Ninth People′s Hospital of Nanhai District,Foshan City from February 2019 to January 2020 and were about to be discharged due to improved condition were selected as the research subjects.According to the random number table method,they were divided into observation group and control group,with 40 cases in each group.The control group received routine health education,while the observation group received health education based on cross-theoretical model.The level of disease self-management,lung function and quality of life were compared between the two groups before intervention and 3 and 6 months after intervention.Results The scores of self-management level and each dimension of quality of life,forced expiratory volume in first second(FEV_(1)),FEV_(1)/forced vital capacity(FEV_(1)/FVC)and the percentage of FEV_(1) in the estimated values(FEV_(1)%pred)of patients in the observation group were higher than those before intervention,and higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion Health education based on transtheoretical model can effectively improve the disease self-management ability of COPD patients,promote the rehabilitation of lung function and improve the quality of life.
