摘要
目的系统评价补肾益智颗粒联合常规药物治疗阿尔茨海默病(AD)的有效性及安全性。方法检索中国知网、万方数据库、维普数据库、Web of Science、中国生物医学文献数据库中补肾益智颗粒及AD的随机对照试验,检索时间为从建库至2020年2月。将符合纳入标准的文献进行基本特征资料的提取及质量评价,并采用RevMan 5.3软件进行Meta分析。结果共纳入文献7篇,均为随机对照试验,共694例病人。Meta分析结果显示:与对照组相比,试验组老年性痴呆评定量表-认知分量表评分降低[MD=-6.63,95%CI(-8.16,-5.09),P<0.00001]、简易智力状态检查量表评分增加[MD=3.73,95%CI(2.09,5.37),P<0.00001],日常生活能力量表评分降低[MD=-3.89,95%CI(-6.01,-1.76),P=0.0003],总有效率提高[RR=1.21,95%CI(1.11,1.32),P<0.0001];且治疗组不良反应发生率低于对照组,差异有统计学意义[RR=0.36,95%CI(0.19,0.69),P=0.002]。结论现有证据表明:补肾益智颗粒联合常规药物较常规药物治疗AD在改善病人认知功能、提高日常生活能力方面较好,且不良反应发生率较低,安全性较好。
Objective To systematically evaluate the efficacy and safety of Bushen Yizhi Granule combined with conventional medicine in the treatment of Alzheimer′s disease(AD)by meta-analysis.Methods China Journal Full-Text Database,Wanfang database,Chinese science and technology periodical database,Chinese Biomedical Literature,Science Citation Index(SCI)for randomized controlled trials in patients were searched from establishment to July 2018 to collect randomized controlled trials of Bushen Yizhi Granule in the treatment Alzheimer′s disease.The literatures were screened based on inclusion and exclusion criteria,and the methodological quality included literatures were assessed.Meta-analysis were used by RevMan 5.3 software.Results A total of 7 literatures were included,involving 694 patients.The result of Meta-analysis showed that,compared with the control group,the score of Alzheimer′s Disease Assessment Scale in the treatment group decreased[MD=-6.63,95%CI(-8.16,-5.09),P=0.00001],Mini-Mental State Examination in the treatment group increased[MD=3.73,95%CI(2.09,5.37),P<0.00001],the score of activities of daily living in the treatment group decreased[MD=-3.89,95%CI(-6.01,-1.76),P=0.0003]and clinical efficacy in the treatment group was better[RR=1.21,95%CI(1.11,1.32),P<0.0001].The number of adverse reactions in the treatment group was lower than that in the control group[RR=0.36,95%CI(0.19,0.69),P=0.002].Conclusion Current evidence showed that compared with the conventional medicine alone,conventional medicine combining with Bushen Yizhi Granule could improve patients′cognitive function,and daily living ability,reduce the incidence of adverse reactions,with better clinical safety.
