摘要
目的探讨补阳还五汤联合针刺对急性脑梗死脑灌注、血同型半胱氨酸(Hcy)和内脏脂肪组织源性丝氨酸蛋白酶抑制剂(Vaspin)水平的影响。方法选择2019年10月至2020年7月泰安市中医二院收治的急性脑梗死患者80例进行前瞻性研究,按照随机数字表法分为两组,每组各40例。对照组实施针刺治疗,观察组则联合应用补阳还五汤,比较干预期间患者语言表达能力,两组治疗后脑灌注指标变化,干预前后两组外周血Hcy水平变化,干预前后两组Vaspin水平变化。结果干预1周后和干预1个月后,观察组语言表达能力得分高于对照组(P<0.05),观察组脑血容量比值(rCBV)及脑血流量比值(rCBF)均显著高于对照组(P<0.05),平均达峰时间比值(rTTP)比值显著低于对照组(P<0.05),干预后观察组Hcy水平低于对照组(P<0.05),干预后观察组Vaspin水平高于对照组(P<0.05)。结论针对急性脑梗死联合使用补阳还五汤,能有效的改善患者发病后语言功能恢复,改善脑灌注,降低Hcy,提高Vaspin水平。
Objective To investigate the impacts of Buyang Huanwu Decoction combined with acupuncture on cerebral perfusion,blood homocysteine(Hcy)and visceral adipose tissue-derived serine protease inhibitor(Vaspin)levels in acute cerebral infarction(ACI).Methods A total of 80 patients with ACI admitted to Second Traditional Chinese Medicine Hospital of Tai'an Hospital from October 2019 to July 2020 were selected for prospective research,and they were divided into the control group(n=40)and the observation group(n=40)according to random number table method.The control group was treated with acupuncture,while the observation group was treated with acupuncture combined with Buyang Huanwu Decoction.The language expression ability of patients during the intervention period,the changes of cerebral perfusion index,the changes of Hcy level in peripheral blood and Vaspin level before and after the intervention were compared between the two groups.Results After 1 week and 1 month of intervention,the scores of language expression ability in the observation group was higher than that in the control group(P<0.05).The cerebral blood volume ratio(rCBV)and cerebral blood flow ratio(rCBF)in the observation group were significantly higher than those in the control group(P<0.05).The ratio of mean time to peak(rTTP)was significantly lower than that in the control group(P<0.05).After the intervention,the Hcy level in the observation group was lower than that in the control group(P<0.05).Meanwhile,the Vaspin level in the observation group was higher than that in the control group(P<0.05).Conclusion Buyang Huanwu Decoction combined with acupuncture can effectively improve the recovery of language function,improve cerebral perfusion,reduce the Hcy and increase the Vaspin levels of patients with ACI.
