负压吸引对肺手术后康复效果的Meta分析 被引量:3

Meta-analysis of the efficacy of negative pressure suction on rehabilitation after lung surgery

摘要 目的系统评价肺部手术后胸腔闭式引流系统接用负压吸引的康复效果,为临床提供循证证据。方法全网检索PubMed、Embase、Web of science、Clinical trail、The Cochrane library、中国生物医学文献数据库、万方数据库、中国知网、维普期刊数据库等。检索时限均为各数据库建库至2020年10月4日。同时借助其他检索。由两名研究人员独立对纳入研究质量评价并提取资料。应用Revman 5.3软件进行Meta分析。结果共纳入18个随机对照试验,包括2854例患者。Meta分析结果显示,肺手术后引流加用负压吸引可降低术后并发症,气胸及术后肺炎的发生率并缩短术后带管时间,差异均有统计学意义(P<0.05)。在漏气时间、迁延漏气发生率、心肺并发症发生率、住院时间方面,差异均无统计学意义(P>0.05)。结论肺手术后胸腔闭式引流系统加用负压吸引在术后并发症、气胸、术后肺炎的发生及术后带管时间方面存在优势,可运用于临床实践。 Objective To systematically evaluate the efficacy of closed thoracic drainage system with negative pressure suction on rehabilitation after lung surgery and provide evidence-based proofs for clinical practice.Methods PubMed,Embase,Web of science,Clinical trial,The Cochrane library,China Biomedical Literature Database,Wanfang Database,China National Knowledge Infrastructure(CNKI),VIP Journal Database,etc.were web-wide retrieved.The retrieval time was from the establishment of each database to October 4th,2020.At the same time with the help of other retrieval.The quality of the included research and extracted data were evaluated independently by two researchers.Meta-analysis was performed by Revman 5.3 software.Results A total of 18 randomized controlled trials were included,involving 2854 patients.The results of meta-analysis showed that the incidence of postoperative complications,pneumothorax and postoperative pneumonia could be reduced and the postoperative time with tube could be shortened by drainage combined with negative pressure suction after lung surgery,with statistically significant differences(P<0.05).There were no statistically significant differences in air leakage time,delayed air leakage rate,cardiopulmonary complications rate and hospitalization time(P>0.05).Conclusion After lung surgery,closed thoracic drainage system combined with negative pressure suction has advantages in reducing incidences of postoperative complications,pneumothorax and postoperative pneumonia and postoperative tube time,which can be used in clinical practice.
