摘要
目的观察口服炎调方对内镜逆行胰胆管造影术后胰腺炎患者的临床疗效。方法94例术后胰腺炎患者随机分为两组,每组47例。对照组给予西医常规保守治疗;治疗组在对照组的基础上加用口服中药炎调方,疗程为7 d。比较两组疗效和并发症发生情况。结果治疗组总有效率稍高于对照组(91.49%vs.80.85%)(P>0.05)。治疗后两组中医证候评分和血清TNF-α、IL-1、IL-2、IL-6水平均较治疗前下降(P<0.05),且治疗组下降更明显(P<0.05)。治疗组血淀粉酶恢复正常时间、腹痛消失时间、首次排便时间和住院天数均短于对照组(P<0.05)。两组均未出现急性呼吸窘迫综合征、胰腺假性囊肿、急性肾功能不全及死亡病例。结论口服炎调方可通过降低术后胰腺炎患者体内多种炎症因子水平,从而下调炎症介质级联反应,加快患者的康复。
Objective To observe the clinical efficacy of oral Yantiao Recipe in the patients with post-endoscopic retrograde cholangiopancreatography pancreatitis(PEP).Methods Ninety-four PEP patients were randomly divided into two groups with 47 cases each.Group C accepted routine conservative treatment with western medicine and group A was treated with additional oral Chinese medicine Yantiao Recipe for seven days.The clinical efficacy and complications were compared between the two groups.Results The overal effectiveness rate of group A was slightly higher than that of group C(91.49% vs.80.85%)(P>0.05).Compared with before, the traditional Chinese medicine symptom score and serum levels of TNF-α,IL-1,IL-2 and IL-6 were decreased after treatment in the two groups(P<0.05),which changed more obviously in group A(P<0.05).The time of blood amylase returning to normal, the time of abdominal pain disappearing, the time of first defecation and the time of hospital stay in group A were shorter than those in group C(P<0.05).There was no aney case with acute respiratory distress syndrome, pancreatic pseudocyst, acute renal insufficiency or death in the two groups.Conclusion Oral Yantiao Recipe can reduce serum levels of inflammatory factors in PEP patients, thereby down-regulating the inflammatory mediators cascade reaction and accelerating the clinical recovery.
