2型糖尿病患者缺血性卒中发病的相关因素研究

Related factors of ischemic stroke in patients with type 2 diabetes

摘要 目的探讨2型糖尿病患者缺血性卒中发病的相关因素。方法回顾性分析2016年1月至2020年1月南平市第一医院收治的261例2型糖尿病患者的临床资料,依据患者是否发生缺血性卒中将患者分为缺血性卒中组(n=50)和非缺血性卒中组(n=211)。单因素和Logistic多因素分析两组患者的临床资料,分析2型糖尿病患者缺血性卒中发病的相关因素。结果经单因素方差分析,非缺血性卒中组和缺血性卒中组患者的年龄、空腹血糖(FBG)、糖化血清蛋白、高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TG)、收缩压、胰岛素抵抗指数、D-二聚体、血同型半胱氨酸水平比较差异有统计学意义(P<0.05)。Logistic多因素分析结果显示,高龄、高HCY、高FBG、高TG、低HDL-C为2型糖尿病患者缺血性卒中发病的危险因素(P<0.05)。结论高龄、高HCY、高FBG、高TG、低HDL-C为2型糖尿病患者缺血性卒中发病的危险因素,临床上应加强对该类患者的检测,降低缺血性卒中的发病率。 Objective To study the related factors of ischemic stroke in patients with type 2 diabetes. Methods The clinical data of 261 patients with type 2 diabetes mellitus admitted to the First Hospital of Nanping from January 2013 to January 2017 were retrospectively analyzed. The patients were divided into ischemic stroke group(n=50) and non-ischemic stroke group(n=211)according to whether they had ischemic stroke. Univariate and Logistic multivariate analyses were performed on the clinical data of patients in the two groups to analyze the related factors of ischemic stroke in patients with type 2 diabetes. Results By one-way anOVA, there were significant differences in age, fasting blood glucose(FBG), glycosylated serum protein, high-density lipoprotein cholesterol(HDL-C), triglyceride(TG), systolic blood pressure, insulin resistance index, D-dimer, and blood homocysteine levels between the non-ischemic stroke group and the ischemic stroke group(P<0.05). Logistic multivariate analysis showed that high age, high HCY, high FBG, high TG and low HDL-C were the risk factors of ischemic stroke in type 2 diabetic patients(P<0.05). Conclusions High age, high HCY, high FBG, high TG and low HDL-C are the risk factors of ischemic stroke in type 2 diabetic patients, and should be intensified to reduce the morbidity of ischemic stroke.
