摘要
目的了解成都市三级医院医务人员参与双向转诊的意愿及影响因素,针对显著性影响因素提出建议,为进一步落实双向转诊提供理论依据。方法采用分层随机抽样方法,对成都市6个区10所三级医院中的208名医务人员进行问卷调查,分析调查对象的基本特征和转诊意愿,采用多因素Logistic回归分析方法分析医务人员参与双向转诊意愿的影响因素。结果83.2%(173/208)的医务人员在患者病情合适的情况下愿意转诊,转诊意愿较高;学历为硕士(91.2%)的医务人员愿意转诊的比例最高。多因素Logistic回归分析显示,了解双向转诊具体流程(OR=0.132,95%CI0.025~0.701)、能提高床位周转(OR=0.295,95%CI0.111~0.783)、所在科室鼓励(OR=0.161,95%CI0.058~0.445)三个因素影响医务人员参与双向转诊的意愿(P<0.05)。结论了解双向转诊具体流程、能提高床位周转和所在科室鼓励三个因素是影响医务人员参与双向转诊意愿的主要因素,应通过建立统一的转诊标准和流程,使双向转诊更加规范化,同时可以通过加强宣传来提高医务人员对双向转诊的认知,并采用一定的激励手段来激发医疗机构和医务人员参与双向转诊的积极性。
Objective To study whether the medical staff in Chengdu tertiary hospital is willing to participate in two-way referral and its influencial factors,and to provide suggestions for influential factors,so as to provide a theoretical basis for further promoting the effective implementation of two-way referral.Methods Randomly selected 208 medical staff in 10 tertiary hospitals in 6 districts of Chengdu to carry out the questionnaire by using a stratified sampling metheod,analyze the basic characteristics of the survey participants and their willingness to refer,and use multi-variate Logistic regression analysis to analyze the influencial factors of medical staff's willingness to participate in two-way referrals.Results 83.2%(173/208)of the medical staff are willing to referral when the patient's condition meet the requirements and the referral intention was higher.Medical personnel with a master's degree(91.2%)are most willing to be referred;multivariate Logistic regression analysis showed that the three factors of understanding the referral process(OR=0.132,95%CI:0.025~0.701),improve bed turnover(OR=0.295,95%CI:0.111~0.783),and the department encourages(OR=0.161,95%CI:0.058~0.445)affecting the willingness of medical staff to participate in two-way referrals.(P<0.05).Conclusion the three factors of understanding the two-way referral process,improving bed turnover,and encouraging three factors in the department are the main factors affecting the willingness of medical staff to participate in two-way referrals.By establishing a unified referral standard and process to make two-way referral more standardized.At the same time,through strengthening publicity to improve medical staff's perception of two-referral,and certain incentives are used to stimulate the enthusiasm of medical institutions and staff to participate in two-way referral.
