摘要
目的:验证自行设计加工的磁锚定装置在结直肠内窥镜下黏膜剥离(ESD)术中应用的可行性。方法:以6个Beagle犬离体结直肠为实验模型,借助微型软组织夹将磁锚定装置中的靶磁体固定在拟切除的病变黏膜边缘,利用锚定磁体吸引牵拉靶磁体完成结直肠ESD术;通过验证手术完成情况、操作的可行性和便捷性以及术中有无副损伤等指标,评价其磁锚定装置在结直肠ESD术中应用的可行性。结果:成功完成了6个结直肠ESD术,内窥镜下利用微型软组织夹可顺利将靶磁体固定于病变黏膜边缘,未出现靶磁体滑落和黏膜组织撕脱的现象。通过调整体外锚定磁体位置可灵活改变靶磁体的牵拉方向和牵拉力,能够清晰显露ESD面并提供足够的组织张力。结论:设计加工的磁锚定装置在结直肠ESD术中具有安全性和可行性。
Objective:To verify the feasibility of self-designed magnetic anchor device in colorectal endoscopic submucosal dissection(ESD).Methods:The isolated colons and rectums of six Beagle dogs’were used as the experimental model,and the target magnet in the magnetic anchor device was fixed to the edge of the diseased mucosa that was prepared for resection with the help of a micro soft tissue clamp,and then the anchor magnet attracted and pulled the target magnet to complete dissection of colorectal mucosa under endoscope.The feasibility of the application of the magnetic anchor device in colorectal submucosal dissection was evaluated by the series of indicators included the completion of the verification surgery,the feasibility and convenience of operation,and the presence or absence of side injuries during the operation.Results:Six colorectal ESDs were successfully completed.The target magnet could be successfully fixed to the edge of the diseased mucosa by using the micro soft tissue clamp under the endoscope,and the phenomenon of target magnet slipping and mucosal tissue avulsion did not occurred.The pulling direction and pulling force of the target magnet can be flexibly changed through adjusted the position of in vitro anchor magnet,which could clearly show the mucosal peeling surface and provide sufficient tissue tension.Conclusion:The designed and manufactured magnetic anchor device has safety and feasibility in colorectal ESD.
