摘要
目的探讨脑室腹腔分流对艾滋病(AIDS)伴新型隐球菌脑膜炎(CM)的治疗效果。方法选取2015年1月至2019年11月本院收治的40例艾滋病合并CM患者,按照随机分配原则分为手术组和对照组,比较两组患者的治疗效果及预后。结果 20例患者先后分别行侧脑室外引流术、脑室腹腔分流术(手术组),20例患者未行手术干预治疗(对照组)。手术组患者均恢复良好,均治愈,病死率为0;对照组患者治愈8例,病死10例,2例无效自动出院。两组治愈和病死的差异有统计学意义(χ^(2)=40.000,13.333,P<0.05)。结论行侧脑室外引流术、脑室腹腔分流术治疗艾滋病合并CM可能是一种值得采取的新方法,可明显降低艾滋病合并CM患者的病死率,改善预后。
Objective To investigate the therapeutic effect of ventriculoperitoneal shunt in the treatment of AIDS patients with cryptococcal meningitis(CM). Methods From January 2015 to November 2019, 40 AIDS patients with CM were selected. They were randomly divided into operation group and control group. 20 patients underwent lateral ventricular drainage and ventriculoperitoneal shunt(operation group), and another 20 patients did not receive surgical intervention(control group). The treatment effect and prognosis of the two groups were compared. Results The 20 patients in the operation group recovered well, with the mortality of zero. In the control group, 8 cases were cured, 10 cases died, 2 cases were discharged because of ineffective treatment. The cure rate of the operation group was significantly higher than that of the control group, and the mortality rate was significantly lower than that of the control group(χ^(2)=40.000, 13.333, P<0.05). Conclusion Ventricle drainage and intraventricularshunt in the treatment of AIDS complicated with CM may be a new method worthy of application, which can significantly reduce the mortality and improve the prognosis of the patients with AIDS combined with CM.
