摘要
目的探讨宫外孕保守失败行腹腔镜手术和急诊手术效果观察。方法选取2018年7月—2019年3月于该院治疗的异位妊娠患者110例为研究对象,前期均行保守治疗,保守治疗失败后行手术治疗,根据手术方式的不同分为观察组和对照组,观察组行腹腔镜手术,对照组行急诊手术。观察两组患者的术中出血量,手术、术后住院以及下床等时间与输卵管再通率等情况。结果观察组手术时间(34.2±4.3)min、术中失血量(522.2±5.2)mL、术后住院时间(3.2±0.3)d以及下床活动时间(5.2±0.4)h均低于对照组手术时间(45.2±4.9)min、术中失血量(611.2±5.8)mL、术后住院时间(7.0±0.8)d以及下床活动时间(17.2±0.9)h,差异有统计学意义(t=11.437、11.319、11.332、11.848,P<0.05);观察组术后7 d的输卵管再通率为91.66%明显高于对照组72.00%,差异有统计学意义(χ^(2)=7.382,P=0.007)。结论在临床工作中,可以放宽异位妊娠的指征,特别是对有生育要求的患者。保守治疗是治疗早期异位妊娠的有效方法。但是我们必须严格筛选这些迹象,一旦出现保守治疗失败的迹象,应立即选择其他治疗方法,腹腔镜手术虽然稍有创伤,但其治愈率较高,能确保患者生命安全。
Objective To investigate the effect of laparoscopic surgery and emergency surgery for conservative failure of ectopic pregnancy.Methods A total of 110 patients with ectopic pregnancy who were treated in the hospital from July 2018 to March 2019 were selected as the research objects.All patients were treated conservatively in the early stage,and surgical treatment was performed after conservative treatment failed.According to different surgical methods,they were divided into observation group and control group.The observation group underwent laparoscopic surgery,and the control group underwent emergency surgery.The intraoperative blood loss,the time of surgery,postoperative hospitalization and getting out of bed,and fallopian tube recanalization rate were observed in the two groups.Results The operation time of the observation group(34.2±4.3)min,intraoperative blood loss(522.2±5.2)mL,postoperative hospital stay(3.2±0.3)d,and get out of bed time(5.2±0.4)h were all lower than those of the control group(45.2±4.9)min,intraoperative blood loss(611.2±5.8)mL,postoperative hospital stay(7.0±0.8)d,and get out of Bed time(17.2±0.9)h,the difference was statistically significant(t=11.437,11.319,11.332,11.848,P<0.05);The fallopian tube recanalization rate in the observation group was 91.66% seven days after surgery was significantly higher than that of the control group 72.00%,the difference was statistically significant(χ^(2)=7.382,P=0.007).Conclusion In clinical work,the indications of ectopic pregnancy can be relaxed,especially for patients with fertility requirements.Conservative treatment is an effective way to treat early ectopic pregnancy.However,we must strictly screen these signs.Once conservative treatment fails,other treatment methods should be selected immediately.Although laparoscopic surgery is slightly traumatic,its cure rate is high,which can ensure the safety of patients'lives and is worthy of promotion.
