摘要
目的采用循证医学的方法探讨精浆硒含量与男性不育症的关系。方法计算机检索中国知网(CNKI)、维普网(VIP)、万方数据库(WanFang)、PubMed、Embase、Cochrane图书馆从建库至2020年8月30日的中、英文文献,筛选出含不育男性和生育男性精浆硒含量对比的病例对照研究。2名研究员独立提取资料和评价文献质量,采用Stata15.1统计软件进行异质性检验、汇总效应量、敏感性分析和发表偏倚检验。结果共检索到370篇文献,最终纳入6篇病例对照研究,合计研究对象847例,其中男性不育症患者425例,健康对照男性422例。meta分析结果显示,不育男性精浆硒含量均数低于正常对照组男性(SMD=-0.63,95%CI=-0.90~-0.37,Z=4.70,P<0.01)。研究间有一定的异质性(I2=60%,P<0.05),敏感性分析表明本研究结果稳定可靠,本文不存在发表偏倚。结论男性不育症患者精浆硒含量降低,可作为男性不育症的监测和评价指标之一。
Objective To investigate the relationship between seminal selenium concentration and male infertility using the method of evidence-based medicine.Methods Case-control studies on comparing the selenium concentration in seminal plasma of infertile and fertile men were screened out by computer by retrieving the Chinese and English literatures from CNKI,VIP,Wanfang Database,PubMed,Embase,and Cochrane libraries from inception to August 30,2020.The data extraction and literature quality assessment were completed independently by two researchers.Stata 15.1 software was used for heterogeneity test,summary effect quantity,sensitivity analysis and publication bias test.Results Six case-control studies in which a total of 847 cases were divided into male infertile(n=425)and control group(n=422)were included definitively out of 370 articles retrieved.The meta-analysis suggested that the mean selenium concentration in seminal plasma of infertile men was lower than the health(SMD=-0.63,95%confidence interval=-0.90~-0.37,Z=4.70,P<0.01).Heterogeneity existed in different studies(I2=60%,P<0.05),while,sensitivity analysis indicated that the results of this study were stable and reliable.There was no publication bias in the article.Conclusion Lower selenium concentration in seminal plasma can be recognized as one of the monitoring and evaluation indicators of male infertility.
