摘要
目的分析二维超声联合四维超声在孕中期胎儿小脑蚓部发育异常诊断中的效能,分析其临床价值。方法随机选取1000例孕中期胎儿产前超声检查孕妇(病例来源:2018年4月—2020年4月本院患者),均行二维超声、四维超声小脑蚓部检查,对于疑似小脑蚓部发育异常病例行核磁共振小脑蚓部检查。以核磁共振检查作为小脑蚓部发育异常诊断金标准,采用SPSS软件绘制二维超声、四维超声、二维联合四维超声的诊断结果 ROC曲线图。结果 1000例孕中期胎儿的超声检查及核磁共振检查诊断结果显示:24例(2.4%)二维超声检查示小脑蚓部发育异常,28例(2.8%)四维超声检查示小脑蚓部发育异常,32例(3.2%)二维超声联合四维超声检查示小脑蚓部发育异常,33例(3.3%)核磁共振检查示小脑蚓部发育异常。二维联合四维超声的诊断敏感性(96.83%)、特异性(94.78%)、准确率(92.81%)高于单纯二维超声(80.18%、79.49%、80.75%)、单纯四维超声(87.79%、86.78%、84.38%)(P<0.05)。结论二维超声联合四维超声在孕中期胎儿小脑蚓部发育异常诊断中具有较佳的敏感性、特异性和准确率,有望作为临床孕中期胎儿小脑蚓部发育异常诊断的有效指标之一。
Objective To analyze the efficacy of two-dimensional ultrasound combined with four-dimensional ultrasound in the diagnosis of fetal cerebellar vermis dysplasia in the second trimester of pregnancy and its clinical value. Methods 1000 pregnant women(source of cases: patients in our hospital from April 2018 to April 2020) were randomly selected for prenatal ultrasound examination of mid-pregnancy fetus. All of them underwent two-dimensional ultrasound and four-dimensional ultrasound examination of cerebellar vermis. For suspected cases of cerebellar vermis dysplasia, magnetic resonance examination of cerebellar vermis was performed. Using magnetic resonance imaging as the gold standard for diagnosis of cerebellar vermis dysplasia, ROC curves of diagnostic results of two-dimensional ultrasound, four-dimensional ultrasound and two-dimensional combined four-dimensional ultrasound were drawn by SPSS software. Results The diagnostic results of ultrasonography and nuclear magnetic resonance in 1000 fetuses during the second trimester of pregnancy showed that 24 cases(2.4%) were examined by two-dimensional ultrasonography, 28 cases(2.8%) by four-dimensional ultrasonography, 32 cases(3.2%) by two-dimensional ultrasonography combined with fourdimensional ultrasonography, and 33 cases(3.3%) by nuclear magnetic resonance. The diagnostic sensitivity(96.83%), specificity(94.78%) and accuracy(92.81%) of two-dimensional ultrasound combined with four-dimensional ultrasound were higher than those of two-dimensional ultrasound alone(80.18%, 79.49%, 80.75%) and four-dimensional ultrasound alone(87.79%, 86.78%, 84.38%)(P<0.05). Conclusion Two-dimensional ultrasound combined with four-dimensional ultrasound has better sensitivity, specificity and accuracy in the diagnosis of fetal cerebellar vermis dysplasia in the second trimester of pregnancy. It is expected to be one of the effective indicators for the diagnosis of fetal cerebellar vermis dysplasia in the second trimester of pregnancy.
