摘要
目的探究右美托咪定复合芬太尼在高血压脑出血开颅手术中的应用效果。方法选取2016-12—2019-12平煤神马医疗集团总医院150例进行高血压脑出血开颅手术的患者,随机分为2组,每组75例。对照组采用丙泊酚复合芬太尼麻醉,观察组采用右美托咪定复合芬太尼麻醉。观察2组患者的血流动力学指标、镇静情况以及不良反应情况。结果钻孔时观察组患者二氧化碳分压(PaCO_(2))、心率(HR)明显低于对照组(P<0.05),血氧分压(PaO_(2))、舒张压(DBP)、收缩压(SBP)明显高于对照组(P<0.05)。观察组镇静良好率89.33%,明显高于对照组的61.33%(P<0.05)。观察组不良反应发生率6.67%,明显低于对照组的29.33%(P<0.05)。结论右美托咪定复合芬太尼在高血压脑出血开颅手术中具有良好的麻醉效果,镇静效果良好,不良反应较少,有利于手术的顺利进行。
Objective To investigate the effect of dexmedetomidine combined with fentanyl in craniotomy for hypertensive cerebral hemorrhage.Methods From December 2016 to December 2019 in our hospital,150 patients undergoing craniotomy for hypertensive cerebral hemorrhage were randomly divided into two groups,75 cases in each group,and one group was the control group.Compound fentanyl anesthesia.The other group is the observation group,anesthetized with dexmedetomidine combined with fentanyl.The hemodynamic indexes,sedation,and adverse reactions of the two groups of patients were observe and compared.Results When drilling,PaCO_(2) and HR of observation group were significantly lower than those of control group(P<0.05),PaO_(2),DBP and SBP of observation group were significantly higher than those of control group(P<0.05).The rate of good sedation in the observation group was 89.33%,which was significantly higher than 61.33%in the control group(P<0.05).The incidence of adverse reactions in the observation group was 6.67%,which was significantly lower than 29.33%in the control group(P<0.05).Conclusion Dexmedetomidine combined with fentanyl has a good anesthesia effect,good sedation effect,and fewer adverse reactions in the craniotomy operation for hypertensive cerebral hemorrhage,which is conducive to the smooth operation of the operation.
