摘要
目的比较膝关节后稳定型膝关节假体(PS型假体)与膝关节后交叉韧带保留型膝关节假体(CR型假体)应用于膝关节置换术治疗中的术后临床疗效。方法随机选取2017年7月—2019年12月在该院行膝关节置换术治疗的患者60例,以应用的膝关节假体类型进行分组,为PS组和CR组,每组30例。PS组患者使用PS假体实施膝关节双间室表面置换术治疗,CR组患者使用CR假体实施膝关节双间室表面置换术治疗。比较两组患者的术后血红蛋白(HGB)减小程度、膝关节最大前后向位移和关节活动度、AKS评分。结果在手术出血量上,两组患者术前的HGB浓度相当,PS组术前(129.96±8.30)g/L,CR组术前(128.03±8.17)g/L,差异无统计学意义(t=0.908,P>0.05);两组患者术后HGB浓度均较术前都有降低,PS组术后(112.00±8.03)g/L,CR组术后(115.06±9.39)g/L,二者水平相当,差异无统计学意义(t=1.357,P>0.05);但PS组术后减小值(17.93±5.58)g/L,CR组术后减小值(12.86±4.24)g/L,CR组患者的HGB减小值显著低于PS组,差异有统计学意义(t=3.952,P<0.05)。在稳定性方面,PS组患者术后3个月时的膝关节最大前后向位移(7.70±1.23)mm,显著小于CR组(9.50±1.52)mm,差异有统计学意义(t=5.024,P<0.05);膝关节活动度上,两组患者术后3个月时的膝关节活动度相当,PS假体(122.16±8.57)°,CR假体(121.33±8.08)°,差异无统计学意义(t=0.387,P>0.05)。两组患者术后3个月时的AKS评分,PS假体术后膝关节评分(90.23±3.15)分,CR假体术后膝关节(89.90±3.13)分,差异无统计学意义(t=0.410,P>0.05)。PS假体术后功能性评分(92.33±5.04)分,CR假体术后功能性评分(92.00±4.84)分,差异无统计学意义(t=0.261,P>0.05)。膝关节评分及功能性评分均相当,差异无统计学意义(P>0.05)。结论比较PS假体和CR假体应用于膝关节双间室表面置换术后的疗效,首先二者均可获得较为理想的膝关节功能和膝关节活动度,这一方面二者不存在明显差异;其�
Objective To compare the postoperative application clinical efficacy of the posterior stable knee prosthesis(PS type prosthesis)and the knee joint posterior cruciate ligament preservation type knee prosthesis(CR type prosthesis)in the treatment of knee arthroplasty.Methods Randomly selected 60 patients who underwent knee arthroplasty in the hospital from July 2017 to December 2019.They were divided into two groups according to the type of knee joint prosthesis used.They were PS group and CR group,with 30 cases in each group.Patients in the PS group were treated with PS prosthesis for double-compartment knee resurfacing,and patients in the CR group were treated with CR prosthesis for double-compartment knee resurfacing.The postoperative hemoglobin(HGB)reduction degree,the maximum anteroposterior displacement of the knee joint,the joint range of motion,and the AKS score were compared between the two groups.Results In terms of surgical blood loss,the preoperative HGB concentrations of the two groups were the same,the PS group was(129.96±8.30)g/L before the operation,and the CR group was(128.03±8.17)g/L before the operation,the difference was not statistically significant(t=0.908,P>0.05);the two groups of patients of the postoperative HGB concentration was lower than that before the operation.The PS group was(112.00±8.03)g/L after the operation,and the CR group was(115.06±9.39)g/L after the operation,the difference was not statistically significant(t=1.357,P>0.05);but the PS group was after the operation.The decrease value was(17.93±5.58)g/L,the postoperative decrease value of the CR group was(12.86±4.24)g/L.The HGB decrease value of the CR group was significantly lower than that of the PS group,the difference was statistically significant(t=3.952,P<0.05).In terms of stability,the maximum anteroposterior displacement of the knee joint in the PS group at 3 months after surgery was(7.70±1.23)mm,which was significantly less than(9.50±1.52)mm in the CR group,the difference was statistically significant(t=5.024,P<
