摘要
目的比较枸橼酸咖啡因注射液与氨茶碱注射液治疗低体重呼吸暂停早产儿的临床疗效及安全性。方法将520例低体重呼吸暂停患儿根据治疗方式分为对照组279例和试验组241例。2组患儿均进行常规治疗。在此基础上,对照组给予静脉滴注氨茶碱注射液,首次负荷量为5 mg·kg^(-1),12 h后改为维持剂量2 mg·kg^(-1)·d^(-1),bid;试验组给予静脉滴注枸橼酸咖啡因,首次负荷量为20 mg·kg^(-1),24 h后改为维持剂量5 mg·kg^(-1)·d^(-1),qd。2组患儿均治疗5~7 d。比较2组患儿的临床疗效、神经功能发育情况,以及药物不良反应的发生情况。结果治疗后,试验组和对照组的呼吸暂停发作次数分别为(0.81±0.52)和(1.51±0.47)time·d^(-1),呼吸暂停消失时间分别为(2.98±1.13)和(4.99±1.42)d,智力发育指数分别为(90.78±2.45)和(81.45±3.04)分,运动发育指数分别为(85.17±3.77)和(80.57±3.54)分,差异均有统计学意义(均P<0.05)。试验组发生的药物不良反应有喂养不耐受43例、胃潴留33例、高血糖32例、腹胀29例和心动过速12例,对照组发生的药物不良反应有胃潴留111例、高血糖102例、腹胀100例、喂养不耐受91例和心动过速77例。2组患儿的上述药物不良反应发生率比较,差异均有统计学意义(均P<0.05)。结论枸橼酸咖啡因注射液治疗低体重呼吸暂停早产儿的临床效果确切,其能显著地促进患儿的神经发育,且安全性较高。
Objective To observe the clinical efficacy and safety of caffeine citrate injection and aminophylline injection in the treatment of premature infants with low weight apnea.Methods A total of 520 children with low weight apnea were divided into control group(n=279 cases)and treatment group(n=241 cases)according to treatment methods.Two groups received conventional treatment.And the control group was given aminophylline injection intravenously,the first loading dose was 5 mg·kg^(-1),after 12 hours,it was changed to a maintenance dose of 2 mg·kg^(-1)·d^(-1),bid.The treatment group was given caffeine citrate intravenously,the first loading dose was 20 mg·kg^(-1),after 24 hours,it was changed to a maintenance dose of 5 mg·kg^(-1)·d^(-1),qd.Two groups were treated for 5-7 days.The clinical efficacy,neurological development and adverse drug reactions were compared between two groups.Results After treatment,the main indexes of treatment and control groups were compared:the number of apnea episodes was(0.81±0.52)and(1.51±0.47)time·d^(-1),the elapsed time of apnea was(2.98±1.13)and(4.99±1.42)d,the mental developmental indexes were(90.78±2.45)and(81.45±3.04)points,the psychomotor developmental indexes were(85.17±3.77)and(80.57±3.54)points,the differences were statistically significant(all P<0.05).The adverse drug reactions of the treatment and control groups were feeding intolerance(43 cases vs 91 cases),gastric retention(33 cases vs 111 cases),abdominal distension(29 cases vs 100 cases),hyperglycemia(32 cases vs 102 cases)and tachycardia(12 cases vs 77 cases).There were significant differences in the incidences of above adverse drug reactions between the two groups(all P<0.05).Conclusion Caffeine citrate injection has a definitive clinical efficacy and safety in the treatment of low birth weight premature infants,which can significantly improve the neurodevelopment.
