摘要
目的比较两种不同钢板固定肱骨干中下段骨折的临床效果及预后情况。方法选择2018年4月至2019年4月在成都新华医院就诊的75例肱骨干中下段骨折患者,其中男性42例,女性33例;年龄20~60岁,平均年龄41.62岁。按照随机数字表法分为单钢板组37例和双钢板组38例。单钢板组接受单钢板内固定治疗,双钢板组接受组合双钢板内固定治疗。比较两组手术相关情况,术后3个月肩关节功能和肘关节功能评分,术后6个月观察骨折愈合效果及并发症发生率。结果双钢板组手术用时、术中出血量、术后引流量、住院时间与单钢板组比较,差异无统计学意义(P>0.05)。双钢板组术后3个月肘关节疼痛、运动功能、稳定性、日常活动评分均高于单钢板组[(35.64±5.89)分vs(30.02±4.86)分,(14.75±4.18)分vs(11.34±3.86)分,(7.21±1.47)分vs(5.42±1.50)分,(19.86±5.37)分vs(16.38±4.78)分。P<0.01];双钢板组术后3个月肩关节疼痛、功能活动、肩关节活动度、肌力评分均高于单钢板组[(7.89±1.56)分vs(6.02±1.39分,(8.02±1.13)分vs(5.61±1.45)分,(7.51±1.24)分vs(5.75±1.34)分,(7.85±1.39)分vs(5.78±1.50)分。P<0.01]。双钢板组术后6个月内骨折愈合率为86.84%,骨折愈合时间为(3.75±1.02)个月,单钢板组分别为83.78%、(4.01±1.13)个月;两组差异无统计学意义(P>0.05)。双钢板组并发症发生率(5.26%)明显低于单钢板组并发症发生率(21.62%)(P<0.05)。结论单钢板与组合双钢板用于内固定治疗肱骨干中下段骨折均能够获得较好手术效果,相比之下组合双钢板能够减少内固定失效,减少并发症,同时对肘关节、肩关节功能有更好的改善作用,更值得推广应用。
Objective Comparison of the clinical effects and prognosis of 2 different plates for fixation of middle and lower humeral shaft fractures.Methods From April 2018 to April 2019,a total of 75 patients with middle and lower humeral shaft fractures were enrolled,which included 42 males and 33 females,aged 20-60 years old with mean age of 41.62 years old.According to random number table,all of them were divided into uni-plate group(n=37,with uni-plate fixation treatment)and dual-plate group(n=38,with dual-plate fixation treatment).The operation-related conditions,shoulder joint function and elbow joint function scores 3 months post operation,and the fracture healing effect and complication rate 6 months post opera-tion between 2 groups were compared.Results There was no statistically significant difference in operation time,intraoperative bleeding,postoperative drainage,and hospitalization between 2 groups(P>0.05).The scores of elbow joint pain,motor function,stability,and daily activities 3 months after operation in dual-plate group were significantly higher than those in uniplate group[(35.64±5.89)scores vs(30.02±4.86)scores,(14.75±4.18)scores vs(11.34±3.86)scores,(7.21±1.47)scores vs(5.42±1.50)scores,(19.86±5.37)scores vs(16.38±4.78)scores.P<0.01];the scores of shoulder joint pain,functional activity,shoulder joint mobility,and muscle strength 3 months post operation in dual-plate group were significantly higher than those in uni-plate group[(7.89±1.56)scores vs(6.02±1.39)scores,(8.02±1.13)scores vs(5.61±1.45)scores,(7.51±1.24)scores vs(5.75±1.34)scores,(7.85±1.39)scores vs(5.78±1.50)scores.P<0.01].The fracture healing rate 6 months post operation in dual-plate group was 86.84%,and fracture healing time was(3.75±1.02)months.The uni-plate group was83.78%and(4.01±1.13)months respectively,and there was no statistical difference between 2 groups.The complication rate of dual-plate group(5.26%)was significantly lower than that of uni-plate group(21.62%)(P<0.05).Conclusion It is demonstrated that uni-pl
