颅内外动脉搭桥术联合BMMNCs治疗缺血性脑卒中疗效观察 被引量:1

摘要 目的:探究颅内-外动脉搭桥术联合骨髓单个核细胞(bone marrow mononuclear cells,BMMNCs)治疗缺血性脑血管病临床疗效。方法:选择于2016年1月—2018年1月间河南大学第一附属医院收治的缺血性脑血管病患者作为研究对象,随机抽出80例,将80例缺血性脑血管病患者随机分为对照组和观察组,每组各40例。对照组给予颅内外动脉搭桥术+血清注射,观察组给予颅内外血管搭桥手术+BMMNCs经搭桥血管注射。对比两组缺血性脑血管病患者术前及术后3个月、6个月、12个月、24个月NIHSS评分和治疗前后梗死脑组织面积。结果:术前两组缺血性脑血管病患者的NIHSS评分对比差异无统计学意义(P>0.05);术后3、6、12、24个月,两组缺血性脑血管病患者的NIHSS评分对比差异均有统计学意义(P<0.05)。治疗前两组缺血性脑血管病患者梗死脑组织面积比较差异无统计学意义(P>0.05),治疗后,观察组患者梗死部位面积缩小程度明显大于对照组(P<0.05)。结论:颅内-外动脉搭桥术联合BMMNCs治疗缺血性脑血管病临床疗效确切,能够明显改善患者梗死情况,减少神经功能损伤,具有较高的应用价值。 Objective:To explore the clinical effect of intracranial-extracranial artery bypass grafting in the treatment of ischemic cerebrovascular disease-Methods:80 patients with ischemic cerebrovascular disease were randomly selected from January2016 to January 2018.They were randomly divided into control group and observation group with 40 cases in each.The control group was treated with intracranial and extracranial artery bypass grafting plus serum injection,while the observation group was treated with intracranial and extracranial artery bypass grafting plus BMMNCs by intracranial and extracranial artery bypass grafting.The NIHSS scores and infarcted brain tissue area before and after treatment were compared between the two groups at 3 months,6 months,12 months and 24 months.Results:There was no statistically significant difference in NIHSS score between the two groups before operation(P>0.05).3,6,12 and 24 months after operation,there was statistically significant difference in NIHSS score between the two groups(P<0.05).There was no statistically significant difference in infarcted brain tissue area between the two groups before treatment(P>0.05).After treatment,the infarcted area of the observation group was significantly smaller than that of the control group(P<0.05).Conclusion:Intracranial-external artery bypass grafting combined with BMMNCs is effective in the treatment of ischemic cerebrovascular disease,which can significantly improve infarction and reduce neurological impairment,and has high application value.
