摘要
患者女,61岁,右下肢肿物2年。皮肤科情况:右小腿伸侧5 cm×8 cm大小肿物,质地中等硬度。皮损组织病理示:真皮内见与周围界限清楚的肿瘤细胞团块,肿瘤细胞主要由大小不等梭形细胞组成,异型性明显,伴大量瘤巨细胞、上皮样细胞及小圆细胞。免疫组织化学染色:CD68、SMA、Vimentin、p53阳性,CD163部分阳性,S-100、CD34、AE1/AE3、EMA、Calponin、Desmin、CD31、Bcl-2阴性,Ki-67约50%。诊断:皮肤未分化肉瘤。
A 61-year-old female presented with a mass on her right lower extremity for two years.Upon clinical examination,a mass(5 cm×8 cm)with medium hardness was observed in the right lower extremity.The histopathological examination found that tumor cell clusters with well-defined boundaries were in the dermis.Tumor cells were mainly composed of spindle cells of different sizes and atypia was obvious.Lots of tumor giant cells and epithelioid cells,round cells were observed.Immunohistochemical analysis was positive for CD68,SMA,Vimentin,p53.Part of tumor cells was positive for CD168,and they were negative for S-100,CD34,AE1/AE3,EMA,Calponin,Desmin,CD31,Bcl-2.The positive rate of Ki-67 was about 50%.The diagnosis was cutaneous undifferentiated sarcoma.
