摘要
目的观察加味八珍汤治疗老年股骨粗隆间骨折术后贫血的临床疗效。方法选取2017年1月—2019年12月收入治疗的符合筛选标准的老年股骨粗隆间骨折PFNA内固定术后贫血患者62例,术后除常规预防感染、抗凝、消肿、止痛治疗外,均接受蔗糖铁注射液静脉点滴治疗,根据入院顺序依据随机数字表法将患者随机分为治疗组与对照组。对照组术后1周内每天静脉点滴蔗糖铁注射液200 mg,治疗组在此基础上口服加味八珍汤,将2组患者治疗后的红细胞计数(RBC)、血红蛋白(HGB)、红细胞压积(HCT)的数值变化及贫血治疗效果和中医临床证候疗效进行对比。结果治疗1周后,治疗组血液指标RBC、HGB、HCT明显高于对照组;对2组患者进行中医证候评分后,除"面色萎黄"一项无明显的差异外(P>0.05),治疗组患者神疲乏力、头晕眼花、气短等症状改善状况优于对照组(P<0.05);治疗组患者贫血改善总体有效率明显高于对照组(P<0.05)。结论基于"益气补血、活血散瘀"理论的加味八珍汤在治疗老年股骨粗隆间骨折术后贫血中疗效明显,可改善气血两虚的症状,加速患者术后康复。
Objective To observe the clinical effect of modified Bazhen decoction in the treatment of anemia after operation of femoral intertrochanteric fracture in the elderly.Methods 62 elderly patients with anemia after PFNA internal fixation of intertrochanteric fracture of femur who met the screening criteria from January 2017 to December 2019 were randomly divided into treatment group and control group according to the admission sequence according to the random number table.The patients in the treatment group were treated with traditional Chinese medicine decoction of modified Bazhen decoction on the basis of intravenous drip of sucrose iron injection.In the control group,200 mg sucrose iron injection was given intravenously every day within 1 week after operation.The numerical changes of RBC,HGB,HCT and the therapeutic effect of anemia were compared between the two groups.Results After one week of treatment,RBC,HGB and HCT in the treatment group were significantly higher than those in the control group.After the TCM syndrome score of the two groups,there was no significant difference between the two groups(P>0.05).The improvement of symptoms such as fatigue,dizziness and shortness of breath in the treatment group was better than that in the control group(P<0.05).The overall efficiency of anemia improvement in the treatment group was significantly higher than that in the control group(P<0.05).Conclusion Modified Bazhen decoction based on the theory of"invigorating qi and blood,activating blood circulation and disperse stasis"has obvious effect in the treatment of anemia after operation of elderly intertrochanteric fracture,which can improve the symptoms of deficiency of both qi and blood and accelerate the recovery of patients after operation.
