摘要
目的:研究行全直肠系膜切除术联合经括约肌间切除术与腹会阴联合切除术治疗低位直肠癌的临床效果。方法:2016年1月~2019年12月于某院治疗的70例低位直肠癌患者双盲法分为对照组和观察组各35例,对照组接受腹会阴联合切除术治疗,观察组接受全直肠系膜切除术联合经括约肌间切除术治疗,研究临床效果。结果:观察组患者手术时间较对照组短,术中出血量较对照组少,手术并发症发生概率为5.71%,低于对照组(P<0.05);两组患者淋巴结检出数、排气时间、恢复进食时间对比无统计意义(P>0.05),两组患者术后2年局部复发率无统计意义(P>0.05);总体生存率观察组高于对照组(P<0.05)。结论:与腹会阴联合切除术治疗对比,全直肠系膜切除术联合经括约肌间切除术治疗低位直肠癌的临床效果更优。
Objective:To study the clinical effect of total mesorectal excision combined with sphincter resection and combined abdominal perineum resection in the treatment of low rectal cancer.Methods:70 patients with low rectal cancer who were treated in a hospital from January 2016 to December 2019 were divided into the two groups by double-blind method.35 patients in the control group received combined abdominal perineum resection,and 35 patients in the observation group received total mesorectal excision combined with sphincter resection,then the clinical effect was studied.Results:Compared with the control group,the operation time of the patients in the observation group was shorter,the amount of intraoperative bleeding was less,and the probability of surgical complications was 5.71%,which was lower than that in the control group(P<0.05).There was no statistically significant difference in lymph node detection,exhaust time and recovery time of eating between the two groups(P>0.05).2 years after operation,the local recurrence rate of the two groups was not statistically significant(P>0.05).The overall survival rate in the observation group was higher than that in the control group(P<0.05).Conclusion:Compared with combined abdominal perineum resection,total mesorectal excision combined with sphincter resection is more effective in the treatment of low rectal cancer.
