摘要
目的探讨肺上皮样血管内皮瘤的CT及^(18)F-FDG PET/CT特点。资料与方法回顾性分析经病理证实的11例肺上皮样血管内皮瘤的CT及^(18)F-FDG PET/CT表现,测量其CT值及最大标准化摄取值(SUVmax)。结果8例行CT平扫,病灶形态均欠规整,呈分叶状或不规则状,病灶内部密度不均,其中5例呈磨玻璃密度,7例病灶内部出现坏死,2例呈实性软组织密度影;5例行CT增强扫描,均表现为不均匀明显强化。6例行^(18)F-FDG PET/CT检查,均呈^(18)F-FDG摄取增高。病灶CT值10~37 Hu,中位值26 Hu;增强扫描CT值49~65 Hu,中位值53 Hu;SUVmax为4.04~5.31,中位值4.45。结论肺上皮样血管内皮瘤病灶CT呈中等强化,SUVmax升高,强化程度及SUVmax介于炎性灶和肺癌之间。CT增强扫描及PET/CT可以为肺上皮样血管内皮瘤的术前诊断提供参考,结合病理检查能够提高其诊断率。
Purpose To explore the CT and ^(18)F-FDG PET/CT features of pulmonary epithelioid hemangioendothelioma.Materials and Methods The CT and ^(18)F-FDG PET/CT manifestations of 11 cases of pulmonary epithelioid hemangioendothelioma confirmed by pathology were retrospectively analyzed,the CT values and maximum standardized uptake value(SUVmax)were measured.Results CT scans were performed in 8 cases,and the lesions were irregularly shaped,showing lobulated or irregular shape.The density inside the lesion was uneven.Among them,5 cases showed ground glass density,and 7 cases appeared necrosis inside the lesion,2 cases showed solid soft tissue density shadow;enhanced scans showed obvious uneven enhancement;5 cases underwent CT enhanced examination,all showed uneven obvious enhancement.6 cases underwent PET/CT examination,and all lesions showed increased ^(18)F-FDG uptake.The CT value of lesions was 10-37 Hu,with median value 26 Hu;enhanced CT value was 49-65 Hu,with median value 53 Hu;SUVmax was 4.04-5.31,with median value 4.45.Conclusion Pulmonary epithelioid hemangioendothelioma lesions are moderately enhanced,SUVmax increases,the degree of enhancement and SUVmax are between inflammatory lesions and lung cancer.Enhanced CT and PET/CT can provide differentiation between P-EHE,inflammatory lesions and lung cancer before surgery,together with pathological examination,it can improve the diagnosis rate of P-EHE.
