七氟烷对冠心病颅脑手术患者行控制性降血压的脑保护效果 被引量:1

摘要 目的研究七氟烷对冠心病颅脑手术患者行控制性降血压的脑保护效果。方法选取2019年1月~2020年5月山东省潍坊市益都中心医院行颅脑手术的52例冠心病患者作为研究对象,将其按照随机数字表法分为试验组和参照组,每组各26例。所有患者于打开硬脑膜后开始进行控制性降血压,试验组给予七氟烷复合吸入,参照组给予硝普钠静脉泵注。比较两组患者手术前后S100钙结合蛋白β(S-100β蛋白)、神经元特异性烯醇酶(NSE)、肌酸激酶同工酶(CK-MB)、肌钙蛋白I(TnI)等水平,并比较两组的术后并发症发生率。结果两组患者手术前后的CK-MB、TnI水平比较,差异无统计学意义(P>0.05);两组患者术后的CK-MB、TnI水平高于本组术前,差异有统计学意义(P<0.05)。试验组患者术后的血清NSE、S-100β蛋白水平低于参照组,差异有统计学意义(P<0.05);且两组患者术后的血清NSE、S-100β蛋白水平高于本组术前,差异有统计学意义(P<0.05);试验组患者的术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论冠心病患者颅脑手术中应用七氟烷行控制性降血压的脑保护效果显著,即七氟烷可对脑组织予以有效保护,可做推广研究。 Objective To study the cerebral protective effect of Sevoflurane on controlled blood pressure reduction in patients with coronary heart disease undergoing craniocerebral surgery.Methods Fifty-two patients with coronary heart disease(CHD)who underwent cranial surgery in Yidu Central Hospital of Weifang City in Shandong Province from January 2019 to May 2020 were selected as the research objects,and they were divided into experimental group and reference group according to random number table method,with 26 cases in each group.All patients underwent controlled blood pressure reduction after opening the dura mater.The experimental group was given Sevoflurane combined inhalation,and the control group was given Sodium Nitroprusside intravenous pump.The levels of S100 calcium binding proteinβ(S-100βprotein),neuron-specific enolase(NSE),creatine kinase isoenzyme(CK-MB),troponin I(TnI)before and after surgery were compared between the two groups,and the incidence of postoperative complications was compared between the two groups.Results There were no significant differences in the levels of CK-MB and TnI between the two groups before and after operation(P>0.05).The levels of CK-MB and TnI in the two groups after surgery were higher than those before surgery,and the differences were statistically significant(P<0.05).The levels of serum NSE and S-100βprotein in experimental group after operation were lower than those in control group,and the differences were statistically significant(P<0.05).The levels of serum NSE and S-100βprotein in the two groups after operation were higher than those before operation,and the differences were statistically significant(P<0.05).The incidence of postoperative complications in experimental group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Sevoflurane can effectively protect the brain tissue by controlling blood pressure reduction in patients with coronary heart disease during craniocerebral surgery,which can be used for furth
