摘要
目的研究全身麻醉与硬膜外麻醉对老年糖尿病患者术后认知功能障碍的影响。方法在该院2016年8月—2019年8月间收治的需要进行腹部手术的老年糖尿病患者中选择100例随机分组,研究组50例采取硬膜外麻醉方式,对照组50例采取全身麻醉方式,比较手术后两组患者的术中麻醉时间及发生认知功能障碍的情况。结果研究组与对照组的术中麻醉时间对比差异无统计学意义(P>0.05);研究组的MMES评分明显高于对照组,且组间对比差异有统计学意义(P<0.05);术后6 h及1周时研究组患者认知障碍的发生率明显低于对照组,且组间对比差异有统计学意义(P<0.05)。结论对于需要进行腹部手术的老年糖尿病患者而言,硬膜外麻醉比全身麻醉对身体产生的不良影响更少,且对人体认知功能产生的损伤也更小,发生认知功能障碍的可能性更低。
Objective To study the effect of general anesthesia and epidural anesthesia on postoperative cognitive dysfunction in elderly diabetic patients.Methods 100 cases of elderly diabetic patients requiring abdominal surgery who were admitted to our hospital from August 2016 to August 2019 were randomly selected.50 cases in the study group were given epidural anesthesia,and 50 cases in the control group were given general anesthesia,compare the intraoperative anesthesia time and the occurrence of cognitive dysfunction of the two groups of patients after surgery.Results The comparison of intraoperative anesthesia time between the study group and the control group was not statistically significant(P>0.05);the MMES score of the study group was significantly higher than that of the control group,and the comparison between groups was statistically significant(P<0.05);At 6 h and one week,the incidence of cognitive impairment in the study group was significantly lower than that in the control group,and the comparison between the groups was statistically significant(P<0.05).Conclusion For elderly diabetic patients who need abdominal surgery,epidural anesthesia has less adverse effects on the body than general anesthesia,and has less damage to human cognitive function,which may cause less cognitive dysfunction.
