摘要
目的对比分析妇科腹腔镜手术中单纯以舒芬太尼静脉麻醉与舒芬太尼联合瑞芬太尼静脉麻醉的临床应用价值。方法该次研究时间范围为2019年3月—2020年2月,抽选于该院接受妇科腹腔镜手术治疗的患者80例作为研究对象,根据入院时编号以单双号法分为两组,对照组单纯以舒芬太尼静脉麻醉,观察组以舒芬太尼联合瑞芬太尼静脉麻醉,分别记录两组患者手术各个时间血压、心率等生理指标变化情况,分析差异性,另外评估对比各组患者术后不同时间疼痛评分与躁动评分。结果两组患者于麻醉前T0时间血压、心率等指标,差异无统计学意义(P>0.05),开始麻醉诱导T1时间血压、心率均出现明显下降,整个手术过程中,观察组患者血压、心率等指标波动幅度明显小于对照组,拔管时T4观察组患者血压(123.23±5.89)mmHg、(74.39±4.23)mmHg、心率(80.84±2.26)次/min均相比对照组较低,差异有统计学意义(t=11.178、16.336、44.206,P<0.05)。另外,拔管后各时间观察组患者疼痛评分(4.13±0.33)分、躁动评分均(2.38±0.22)分明显低于对照组,差异有统计学意义(t=41.246、43.319,P<0.05)。结论妇科患者接受腹腔镜手术时选择舒芬太尼联合瑞芬太尼静脉用药方式麻醉效果良好,对患者影响小,安全性高。
Objective To compare and analyze the clinical value of intravenous anesthesia with sufentanil alone and sufentanil combined with remifentanil in gynecological laparoscopic surgery.Methods The study time range is from March 2019 to February 2020.80 patients who received gynecological laparoscopic surgery in the hospital were selected as the research objects.According to the number at the time of admission,they were divided into two groups by odd and even numbers.The control group was intravenously anesthetized with sufentanil alone,and the observation group was anesthetized with sufentanil combined with remifentanil.The changes in blood pressure and heart rate and other physiological indicators of the two groups were recorded at each time of the operation,the differences were analyzed,and the comparison of pain score and agitation score of patients in each group at different time after operation was evaluated.Results There was not statistical signiticant difference between the two groups of patients in blood pressure and heart rate at T0 time before anesthesia(P>0.05).The blood pressure and heart rate at T1 time after induction of anesthesia were significantly reduced.During the entire operation,the observation group had blood pressure and heart rate indicators.The fluctuation range was significantly smaller than that of the control group.At the time of extubation,the blood pressure(123.23±5.89)mmHg,(74.39±4.23)mmHg,and heart rate(80.84±2.26)times/min of the patients in the T4 observation group were lower than those of the control group,the difference was statistically significant(t=11.178,16.336,44.206,P<0.05).In addition,the pain score(4.13±0.33)points and restlessness score(2.38±0.22)points of the observation group were significantly lower than those of the control group,the difference was statistically significant(t=41.246,43.19 P<0.05)at each time after extubation.Conclusion When gynecological patients undergo laparoscopic surgery,sufentanil combined with remifentanil intravenously has a good anesthesia
