摘要
目的探讨莲芪胶囊联合超声引导下冷循环射频消融术(LCRFA)治疗老年胆囊癌肝转移的临床疗效。方法选取2010年1月~2015年5月安康市中医医院收治的114例老年不可切除的胆囊癌肝转移患者,随机分为观察组及对照组,每组57例。研究对象均采用LCRFA进行治疗,观察组在此基础上给予莲芪胶囊治疗,治疗时间均为3个月。比较两组近期疗效和远期疗效,比较治疗前后两组患者肝功能指标和免疫功能指标,比较两组患者的术后并发症情况。结果观察组客观缓解率及疾病控制率均高于对照组,差异有统计学意义(P<0.05);治疗后观察组丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)、天冬氨酸氨基转移酶(AST)及碱性磷酸酶(ALP)均低于对照组(P<0.05),IgG、IgA、IgM、CD3+及CD4+均高于对照组(P<0.05);术后疼痛、发热、出血、感染、黄疸及胸腔积液发生率与对照组比较差异无统计学意义(P>0.05)。观察组1年生存率、2年生存率、无疾病进展生存时间(PFS)和总生存时间(OS)均高于对照组(P<0.05)。结论莲芪胶囊联合LCRFA治疗老年不可切除的胆囊癌肝转移疗效较好,且可有效改善患者免疫功能,延长患者生存时间,值得进行深入研究。
Objective To study the clinical efficacy of lianqi capsule combined with ultrasound laparoscopic cool-tip radiofrequency ablation(LCRFA)in the treatment of older patients with liver metastasis of gallbladder cancer.Methods 114 older patients with unresectable hepatic metastasis of gallbladder cancer in Ankang City Hospital of Traditional Chinese Medicine from January 2010 to May 2015 were randomly divided into the observation group and the control group,57 cases in each group.The control group was treated with LCRFA,and the observation group was treated with LCRFA plus lianqi capsule.The patients were treated for 3 months.Short-term effect and long-term effect were evaluated,the levels of liver function indicators and immune function indicators were detected.The complications were documented.Results The objective response rate and disease control rate in the observation group were higher than those in the control group,there were significant differences between the two groups(P<0.05).After treatment,the levels of alanine aminotransferase(ALT),total bilirubin(TBIL),aspartate aminotransferase(AST)and alkaline phosphatase(ALP)in the observation group were significantly lower than those in the control group(P<0.05).The level of IgG,IgA,IgM,CD3+and CD4+in the observation group were significantly higher than those in the control group(P<0.05).There were no significant differences in the incidences of pain,fever,bleeding,infection,jaundice and pleural effusion after the treatment between the two groups(P>0.05).The 1-year survival rate,2-year survival rate,progression-free survival and overall survival in the observation group were significantly higher than those in the control group(P<0.05).Conclusion The lianqi capsule combined with LCRFA has a great effect on unresectable liver metastasis of gallbladder cancer in older patients.It might improve the immune function and prolong the survival time of the patients.
