摘要
目的探讨社区精神分裂症患者服药信念现状及影响因素。方法随机抽取国家严重精神障碍信息系统(新乡市)中的289例精神分裂症患者为研究对象,采用自制一般资料问卷、阳性与阴性症状量表、社会功能缺陷筛选量表、精神分裂症认知功能成套测验量表和简明药物影响与信念量表调查入组患者的一般资料、认知功能、社会功能和服药信念。分析影响患者服药信念的影响因素。结果不同管理情况、宗教信仰、规律服药、危险倾向、年龄、病程、家庭类型、婚姻状态、职业、文化程度、病种患者服药信念比较差异有统计学意义(P<0.01);积极信念患者阳性与阴性症状量表、社会功能缺陷筛选量表评分显著低于消极信念患者(P<0.01),精神分裂症认知功能成套测验量表评分显著高于消极信念患者(P<0.01)。管理情况、宗教信仰、职业、受教育程度、阳性与阴性症状量表阴性症状评分及一般精神病理症状评分、精神分裂症认知功能成套测验量表评分对患者服药信念有显著影响(P<0.05或0.01)。结论社区精神分裂症患者服药信念现状较差,患者管理情况、宗教信仰、职业、受教育程度等是其影响因素,应给予重视。
Objective To explore the current status and influencing factors of community schizophrenic patients'belief in taking medication.Methods 289 patients with schizophrenia were randomly selected from National Register Information System for Severe Mental Disorders in Xinxiang.The self-made general information questionnaire,positive and negative symptom scale(PANSS),social disability screening schedule(SDSS),MA-TRICS consensus cognitive battery(MCCB),brief evaluation of medication influence and beliefs(BEMIB)and beliefs scale were used to investigate the general information,cognitive function,social function and medication belief of the enrolled patients.Analyze the influencing factors of patients'belief in taking medication.Results There were statistically significant differences among patients with different management conditions,religious beliefs,regular medication,risk propensity,age,course of disease,family type,marital status,occupation,education level,and disease types(P<0.01).The PANSS and SDSS scores of patients with positive beliefs were significantly lower than those with negative beliefs(P<0.01),and the scores of MCCB were significantly higher than those with negative beliefs(P<0.01).Management status,religious beliefs,occupation,education level,PANSS negative symptom score,general psychopathological symptom score,and MCCB score had significant impacts on patients'belief in taking medication(P<0.05 or 0.01).Conclusions The current status of community schizophrenic patients'belief in taking medication is poor.Patient management situation,religious beliefs,occupation, education level, etc. are influencing factors, which should be paid attention to.
