摘要
目的探讨不同中医证型急性脑梗死(ACI)患者血清同型半胱氨酸(Hcy)、缺血修饰白蛋白(IMA)水平,并分析其与神经功能缺损程度及短期预后的关系。方法选取2017年6月—2019年12月确诊的ACI患者168例作为观察组,再选取同期健康体检者80例作为对照组。将观察组根据不同中医证型分为阴虚风动型(46例),风火上扰型(58例),风痰瘀阻型(64例)3个亚组。分别检测各组血清Hcy、IMA水平。采用Pearson相关分析观察组血清Hcy、IMA水平与其神经功能缺损程度及短期预后的相关性。结果观察组Hcy、IMA水平均高于对照组,差异均有统计学意义(P<0.05)。风痰瘀阻型患者Hcy、IMA水平均高于阴虚风动型和风火上扰型(P<0.05),差异均有统计学意义。Pearson相关分析显示,ACI患者血清Hcy、IMA水平与神经功能缺损程度(NIHSS分数)之间均呈正相关(r=0.735,r=0.786,P<0.001)。Pearson相关分析显示,ACI患者血清Hcy、IMA水平与短期预后情况(mRS评分)之间均呈正相关(r=0.907,r=0.889,P<0.001)。结论ACI患者血清Hcy、IMA水平升高。各证型中,风痰瘀阻型患者血清Hcy、IMA水平明显高于其他证型。ACI患者血清Hcy、IMA水平与其神经功能缺损程度及短期预后情况密切相关。血清Hcy、IMA水平可以作为诊断ACI神经功能缺损程度和短期预后的指标。
Objective To explore the levels of serum homocysteine(Hcy)and ischemia-modified albumin(IMA)in patients with acute cerebral infarction(ACI)of different TCM syndromes,and analyze their relationship with the degree of neurological deficit and short-term prognosis.Methods 168 patients with ACI diagnosed in our hospital from June 2017 to December 2019 were selected as the observation group,and 80 healthy medical examines during the same period were selected as the control group.According to different TCM syndromes,the observation group was divided into 3 subgroups:Yin-deficiency and wind-acting type(n=46),wind-fire upset type(n=58),and wind phlegm stasis type(n=64).The serum Hcy and IMA levels of each group were detected.Pearson correlation analysis was used to observe the correlation of serum Hcy and IMA levels with the degree of neurological deficit and short-term prognosis.Results The levels of Hcy and IMA in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of Hcy and IMA in patients with wind-phlegm and blood stasis type were higher than those in Yin-deficiency and wind-acting type and wind-fire upset type,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the serum Hcy and IMA levels in ACI patients were positively correlated with the degree of neurological deficit(NIHSS score)(r=0.735,r=0.786,P<0.001).Pearson correlation analysis showed that serum Hcy and IMA levels in ACI patients were positively correlated with short-term prognosis(mRS score)(r=0.907,r=0.889,P<0.001).Conclusion The serum Hcy and IMA levels of patients with ACI increased,and the serum Hcy and IMA levels of patients with wind phlegm stasis type were significantly higher than those of other syndrome types.The levels of serum Hcy and IMA in ACI patients are closely related to the degree of neurological deficit and short-term prognosis.Serum Hcy and IMA levels can be used as indicators to diagnose the degree of AC
