下肢等速肌力训练对中重度COPD患者运动能力和生活质量的影响 被引量:11

Effect of isokinetic strength training on physical ability and quality of life in patients with moderate or severe chronic obstructive pulmonary disease

摘要 目的观察下肢等速肌力训练对中重度慢性阻塞性肺疾病(COPD)患者生活质量和运动能力的影响。方法随机对照研究。选取上海市浦东新区公利医院2017年3月至2019年3月COPD稳定期患者(Ⅱ、Ⅲ级)40例,采用随机数字表法分为对照组和运动组,每组20例。对照组给予常规药物治疗和疾病健康教育。运动组在对照组干预基础上行12周下肢等速肌力训练。2组于干预前后测定膝关节肌群肌肉功能、肺功能、6分钟步行距离、Borg呼吸困难评分、汉密顿焦虑量表、汉密顿抑郁量表和慢性阻塞性肺疾病评估测试(CAT)。结果与对照组干预后相比,运动组干预后6分钟步行距离显著增加(t=-2.567,P<0.05),而CAT评分、Borg呼吸困难评分、汉密顿抑郁评分和汉密顿焦虑评分均显著降低(P值均<0.05)。与运动组干预前相比,运动组干预后6分钟步行距离显著增加(t=-9.667,P<0.05),而CAT评分、Borg呼吸困难评分、汉密顿抑郁评分和汉密顿焦虑评分均显著降低(P值均<0.05)。与运动组干预前相比,运动组干预后双下肢伸肌群峰力矩、相对峰力矩、总功均显著升高(P值均<0.05)。与对照组干预前相比,对照组干预后右下肢伸肌群峰力矩显著降低(t=-2.699,P<0.05),左下肢伸肌群峰力矩、相对峰力矩均显著降低(P值均<0.05)。结论下肢等速肌力训练可以减轻中重度COPD患者气促程度及负性情绪,提高运动能力及日常生活质量,可以作为肺康复的一种有效的方法。 Objective To observe the effect of isokinetic strength training on the quality of life and motor ability of patients with chronic obstructive pulmonary disease(COPD).Methods A total of 40 patients with stable COPD(gradeⅡandⅢ)in Gongli Hospital,Shanghai Pudong New Area from March 2017 to March 2019 were randomly divided into control group(n=20)and exercise group(n=20).The control group was given routine drug treatment and health education.The exercise group was given 12 weeks of isokinetic muscle strength training on the basis of the control group.Muscle function,pulmonary function,6-minute walking distance(6MWD),Borg dyspnea score,Hamilton depression scale(HAMD),Hamilton anxiety scale(HAMA)and COPD assessment test(CAT)were performed before and after intervention in both groups.Results Compared with the control group after intervention,6MWD in exercise group was significantly increased(t=-2.567,P<0.05),while CAT score,Borg dyspnea score,HAMD and HAMA were significantly decreased(all P<0.05).Compared with that before the intervention,6MWD in the exercise group was significantly increased(t=-9.667,P<0.05),while the CAT score,Borg dyspnea score,HAMD and HAMA were significantly decreased(all P<0.05).After intervention,the peak torque,relative peak torque and total power of extensors of both lower limbs in exercise group were significantly increased(all P<0.05).Compared with the control group before intervention,the peak torque of right lower limb extensors in the control group was significantly decreased(t=-2.699,P<0.05),and the peak torque and relative peak torque of left lower limb extensors were significantly decreased(both P<0.05).Conclusions Isokinetic strength training-based pulmonary rehabilitation significantly improves treatment outcomes in Chinese patients with severe COPD.
