摘要
目的:探究CT尿路造影在输尿管软镜治疗肾结石的精准应用效果。方法:选择本院2019年9月~2020年11月于本院接受治疗的肾结石患者100例,按随机数字表法分为两组各50例。研究组于输尿管软镜治疗前行CT尿路造影检查,对照组于输尿管软镜治疗前行X线静脉泌尿系统造影检查。对比两组围术期指标,结石清除率、手术成功率及并发症情况。结果:研究组手术时间为(76.38±12.63)min、术后下床活动时间为(2.71±0.70)d、导管拔除时间为(1.60±0.18)d、住院时间为(5.18±1.36)d均短于对照组的(142.64±32.16)min、(4.39±0.82)d、(2.36±0.24)d、(7.25±2.27)d,术中出血量为(85.07±10.63)mL少于对照组的(96.88±12.45)mL,差异有统计学意义(P<0.05);研究组结石清除率为96.00%、手术成功率为98.00%,均高于对照组的80.00%、84.00%,并发症发生率为4.00%,低于对照组的16.00%,差异有统计学意义(P<0.05)。结论:CT尿路造影在输尿管软镜治疗肾结石应用中能够明确肾结石与动脉间的空间结构,为手术治疗提供参考依据,利于提高结石清除率,并发症少,安全性高。
Objective:To explore the accurate application of CT urography in the treatment of renal calculi with flexible ureteroscope.Methods:100 patients with renal calculi treated in our hospital from September 2019 to November 2020 were randomly divided into two groups:50 patients in each group(n=50).CT urography was performed in the study group before flexible ureteroscope treatment,while X-ray intravenous urography was performed in the control group before flexible ureteroscope treatment.The perioperative indexes,stone clearance rate,success rate of operation and complications were compared between the two groups.Results:The time of operation(76.38±12.63)min,the time of getting out of bed after operation(2.71±0.70)d,the time of catheter removal(1.60±0.18)d and the time of hospitalization(5.18±1.36)d in the study group were shorter than those in the control group(142.64±32.16)min,(4.39±0.82)d,(2.36±0.24)d,(7.25±2.27)d,and the amount of intraoperative bleeding(85.07±10.63)mL in the study group was less than that in the control group(96.88±12.45)mL,(P<0.05).The stone clearance rate of the study group was 96.00%,the success rate of operation was 98.00%,which was higher than 80.00%and 84.00%of the control group,and the incidence of complications was 4.00%,which was lower than that of the control group(16.00%).The difference was statistically significant(P<0.05).Conclusion:CT urography can clearly define the spatial structure between renal calculi and arteries in the treatment of renal calculi with soft ureteroscope,which provides a reference basis for surgical treatment,and is beneficial to improve the stone clearance rate with few complications and high safety.
