摘要
目的:探讨全身振动训练对特重度烧伤患者骨骼的影响.方法:康复期特重度烧伤患者30例随机分为观察组和对照组各15例,均采用综合康复疗法,包括运动治疗(传统肌力训练、牵伸治疗、关节松动训练、步行训练等)、物理因子治疗等、水疗、压力治疗等,1次/d,6d/w,共12周.观察组受试者另接受全身振动训练,1次/d,6d/w,持续12周.治疗前、后使用外周定量CT(pQCT)测试2组受试者左侧胫骨干骺端骨量(全骨矿物质总含量和松质骨矿物质含量)、骨形态计量学(横断面总面积和松质骨横断面总面积)、容积密度(总密度和骨小梁密度)指标,和骨干骨量(全骨矿物质总含量和密质骨矿物质含量)、骨形态计量学(横断面总面积、密质骨横断面总面积和骨髓腔面积)、容积密度(总密度和密质骨密度)指标.结果:(1)2组受试者左下肢胫骨干骺端松质骨矿物质含量,松质骨密度均降低,但全身振动训练减少观察组受试者胫骨干骺端松质骨矿物质含量的丢失,缓解其骨密度降低(P<0.01),2组受试者全骨矿物质总含量、横断面总面积、松质骨横断面总面积、总密度数据比较均无显著性意义(P>0.05).(2)2组受试者左下肢胫骨骨干密质骨横断面总面积均增加,且全身振动训练有效增加观察组受试者胫骨骨干密质骨横断面总面积(P<0.01);2组受试者全骨矿物质总含量、密质骨矿物质含量、横断面总面积、骨髓腔面积、总密度和密质骨密度数据比较均无显著性意义(P>0.05).结论:全身振动训练有助于缓解特重度烧伤患者松质骨骨密度降低,减少松质骨骨质丢失,增加密质骨横断面总面积,值得临床推广.
Objective:To explore the effect of whole body vibration training on the bone of patients with severe bum.Methods:Thirty patients with severe bum in rehabilitation phase were randomly divided into experimental group and control group with 15 cases in each group.All patients were treated with comprehensive rehabilitation therapy,including kinesiotherapy(muscle strength training,stretching,mobilization training,walking training,etc.),electrical physiotherapy,hydrotherapy,pressure therapy,once a day,six days a week,for 12 weeks.The subjects in the experimental group received the whole body vibration training,once a day,six days a week,for 12 weeks.Before and after training,peripheral quantitative CT(pQCT)was used to test the corresponding indexes of left tibial metaphysis of the two groups,including bone mass(total bone mineral content and trabecular bone mineral content),bone morphometry(total cross-sectional area and total trabecular cross-sectional area),volume density(total density and trabecular density)in the tibial metaphysis,and bone mass(total bone mineral content and cortical bone mineral content),bone morphometry(total cross-sectional area,total cross-sectional area of cortical bone and area of marrow cavity),volume density(total density and cortical bone density)in the tibial shaft.Result:①The content of trabecular bone mineral and the density of trabecular bone in the left tibial metaphysis of the two groups are both decreased.Compared with the control group,the whole body vibration training significantly reduced the loss of trabecular bone mineral content,and alleviated the decrease of trabecular bone mineral density in the metaphysis of the tibial metaphysis in the experimental group(P<0.01).There was no significant difference in the tibial metaphysis of total bone mineral content,total cross-sectional area,total trabecular cross-sectional area and total density between the two groups(P>0.05).②The total cross-sectional area of cortical bone in the tibial shaft of the two groups increased,and the to
