摘要
目的:分析总结溃疡性结肠炎(UC)的MSCT表现,以提高其诊断准确率,为临床选择正确的治疗方案提供依据。方法:回顾性分析26例UC患者的MSCT资料。结果:26例中,7例仅累及直肠、乙状结肠,13例累及直肠、乙状结肠和降结肠,6例为全结肠UC。肠壁增厚25例,厚度5~10 mm,平均(6.5±2.1)mm。16例病变肠壁浆膜面光滑,10例浆膜面周围可见细小条索影;21例病变肠黏膜明显强化;12例肠黏膜面多发结节样、丘状突起;10例显示肠黏膜下气泡。14例出现肠壁分层;9例肠腔狭窄、结肠袋消失、肠管缩短变形;17例肠系膜密度增加;15例肠系膜血管增多增粗;7例肠系膜淋巴结增大。结论:MSCT可清晰显示UC患者肠壁、肠系膜血管情况及肠外表现,明确疾病范围和严重程度,判断病变的时期(活动期或缓解期),在UC的诊断、评估病情、制订个体化治疗方案、评价疗效及判定预后等方面具有重要价值。
Objective:MSCT findings of ulcerative colitis were analyzed to improve the diagnostic accuracy and to provide the basis for choosing the right treatment plan.Methods:MSCT imaging data of 26 patients with ulcerative colitis confirmed clinically were retrospectively analyzed.Results:In 26 cases of ulcerative colitis,only the rectum and sigmoid colon were involved in 7 cases,the rectum and sigmoid colon and descending colon in 13 cases and the whole colon in 6 cases.There were 25 cases of intestinal wall thickening,reached to 5~10 mm,average of(6.5±2.1)mm.In 16 cases,the serosal surface of the intestinal wall were smooth,and 10 cases had small cord shadow around the serosal surface.There were 21 cases of increased intestinal mucosal enhancement,12 cases of intestinal mucosal surface uneven,10 cases of intestinal submucosal bubbles,14 cases of intestinal wall stratification,9 cases of intestinal stenosis,disappearance of colonic pouch,shortening of intestinal canal,17 cases of mesenteric density increased,15 cases of mesenteric blood vessels increased and enlarged.Mesenteric lymph nodes enlarged in 7 cases.Conclusions:MSCT can clearly display the intestinal wall,mesenteric blood vessels and extraintestinal manifestations of ulcerative colitis patients,clarify the scope of disease involvement,severity and determine the stage of disease(active or remission).It has important application value in the diagnosis of ulcerative colitis,evaluation of disease condition,formulation of individualized treatment plan,evaluation of curative effect and judgment of prognosis.
