摘要
目的:探讨高渗盐液治疗严重颅脑损伤并发休克患者对脑灌注压、格拉斯哥昏迷评分等指标的影响。方法:2019年2月-2020年6月收治严重颅脑损伤并发休克患者66例,随机分为两组,各33例。对照组采用甘露醇+林格氏液治疗;观察组采用高渗盐液治疗。比较两组格拉斯哥昏迷评分、疗效和临床相关指标。结果:观察组死亡率及植物生存率均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组平均动脉压、脑灌注压、颅内压及尿量水平均优于对照组,差异有统计学意义(P<0.05)。结论:高渗盐液治疗严重颅脑损伤并发休克患者提高了临床疗效,减少植物生存率、死亡率,且可显著改善平均动脉压、脑灌注压、颅内压、尿量、格拉斯哥昏迷评分。
Objective:To investigate the effect of hypertonic saline on cerebral perfusion pressure and Glasgow coma score in patients with severe craniocerebral injury complicated with shock.Methods:From February 2019 to June 2020,66 patients with severe craniocerebral injury complicated with shock were selected,they were randomly divided into the two groups with 33 cases in each group.The control group was treated with mannitol and Ringer's solution.The observation group was treated with hypertonic saline.The Glasgow coma score,curative effect and clinical related indexes of the two groups were compared.Results:The mortality and plant survival rate in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05).The total effective rate in the observation group was significantly higher than that in the control group,the difference was statistically significant(P<0.05).The levels of mean arterial pressure,cerebral perfusion pressure,intracranial pressure and urine volume in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:In patients with severe craniocerebral injury complicated with shock,hypertonic saline treatment can improve clinical efficacy,reduce plant survival rate and mortality,and significantly improve mean arterial pressure,cerebral perfusion pressure,intracranial pressure,urine volume and Glasgow coma score.
