摘要
目的探讨一期双侧全膝关节置换与单侧全膝关节置换手术安全性和有效性。方法回顾性分析2019年1月1日至12月31日在本院行全膝关节置换的患者共96例,其中一期行双侧全膝关节置换术患者36例(双侧组),行单侧全膝关节置换术患者60例(单侧组)。比较两组术前、术后血红蛋白和红细胞计数,术前、术后第1、3、15天疼痛视觉模拟(VAS)评分和术前、术后第3、15天膝关节HSS评分,术后并发症发生率,住院天数。结果双侧组术后血红蛋白水平和红细胞计数下降较单侧组明显[(107.09±15.50)g/L比(113.43±13.84)g/L,t=2.078,P=0.040;(3.53±0.56)×10^(12)/L比(3.80±0.53)×10^(12)/L,t=2.388,P=0.019)]。两组术后VAS评分、膝关节HSS评分、术后并发症发生率以及住院天数差异无统计学意义(均P>0.05)。结论加强围手术期血常规和血红蛋白监测,一期双侧和单侧全膝关节置换安全性和术后疗效差异无统计学意义。
Objective To explore the efficacies and safety of one-stage total knee replacement for one versus both knees.Methods Ninety-six patients who took one-stage total knee replacement from January 1 to December 31,2019 at our hospital were retrospectively analyzed,including 36 cases on both knees,a both sides group,and 60 on one knee,a one side group.The hemoglobin levels and red blood cell counts before and after the operation,VAS scores before and day 1,3,and 15 after the operation,the HSS scores before and day 3 and 15 after the operation,incidence of complications after the operation,and hospital stay were compared between these two groups.Results After the operation,the hemoglobin level and red blood cell count were lower in the both sides group than in the one side group[(107.09±15.50)g/L vs.(113.43±13.84)g/L,t=2.078,P=0.040;(3.53±0.56)×1012/L vs.(3.80±0.53)×1012/L,t=2.388,P=0.019].There were no statistical differences in the VAS scores,HSS scores,incidence of complications after operation,and hospital stay between these two groups(all P>0.05).Conclusion There are no statistical differences in the safety and postoperative efficacy between one-stage total knee replacement on both knees and on one,but perioperative blood routine and hemoglobin level should be monitored.
