摘要
目的探讨完全腹腔镜技术在门静脉高压巨脾切除术中的应用价值。方法选择2019年3月—2020年3月高安市人民医院收治的40例乙肝肝硬化门静脉高压巨脾患者,按照随机数字表法分为试验组与对照组,各20例。试验组行完全腹腔镜下巨脾切除术治疗,对照组行开腹巨脾切除术治疗,对比2组手术相关指标、术后恢复情况。结果试验组术中出血量、切口长度、术后切口疼痛程度分别为(307.75±108.47)mL、(8.44±1.69)cm、(2.51±0.78)分,低于对照组的(457.18±154.65)mL、(17.20±4.58)cm、(4.32±1.25)分,差异有统计学意义(P<0.05);2组手术时间相比,差异无统计学意义(P>0.05)。试验组肠功能恢复时间、术后拔管时间、住院时间分别为(2.35±0.64)d、(2.94±0.89)d、(6.42±1.36)d,短于对照组的(4.69±0.78)d、(3.66±0.94)d、(8.02±1.43)d,有统计学差异(P<0.05);与对照组相比,试验组术后并发症发生率更低,有统计学差异(P<0.05)。结论门静脉高压巨脾切除术中使用完全腹腔镜技术具有创伤小、恢复快、安全性高等优点,可有效改善患者预后。
Objective To investigate the application value of total laparoscopic technique in splenectomy for portal hypertension.Methods From March 2019 to March 2020,40 cases of hepatitis B cirrhosis with portal hypertension and splenomegaly were randomly divided into experimental group and control group,20 cases in each group.The experimental group was treated with total laparoscopic splenectomy,while the control group was treated with open splenectomy.The operation related indexes and postoperative recovery of the two groups were compared.Results The intraoperative blood loss,incision length and postoperative incision pain in the experimental group were(307.75±108.47)ml,(8.44±1.69)cm,(2.51±0.78)points,which were lower than(457.18±154.65)ml,(17.20±4.58)cm,(4.32±1.25)points in the control group,the difference was statistically significant(P<0.05);there was no significant difference in the operation time between the two groups(P>0.05)The recovery time of intestinal function,postoperative extubation time and hospitalization time were(2.35±0.64)d,(2.94±0.89)d,(6.42±1.36)d,which were shorter than those of the control group(4.69±0.78)d,(3.66±0.94)d,(8.02±1.43)d,with statistical difference(P<0.05);compared with the control group,the incidence of postoperative complications in the experimental group was lower,with statistical difference(P<0.05).Conclusion The application of total laparoscopic technique in splenectomy for portal hypertension has the advantages of small trauma,quick recovery and high safety.
