摘要
目的探讨KTH整合式护理在中晚期食管癌患者经外周静脉穿刺中心静脉置管化疗中的应用效果.方法将120例中晚期食管癌患者按照住院时间分为两组,各60例.两组均给予常规护理干预,观察组在此基础上给予KTH整合式护理干预.干预前后采用一般自我效能感量表评定两组的自我效能感,自护能力测定量表评定两组自护能力,欧洲癌症治疗与研究组织生活质量量表核心量表评定两组生活质量,采用自制导管管理知识掌握度评估问卷评定两组导管管理知识掌握情况,比较两组并发症发生率,采用自制护理满意度评估问卷评定两组护理满意度.结果干预后两组一般自我效能感量表及自护能力测定量表评分均较干预前显著升高(P<0.01),观察组较对照组升高更显著(P<0.01).干预后观察组欧洲癌症治疗与研究组织生活质量量表核心量表情绪、认知、社会、总体状况评分均显著高于干预前(P<0.01),对照组认知评分显著高于干预前(P<0.01),观察组认知、社会、总体状况评分较对照组升高更显著(P<0.05或0.01).观察组导管管理知识掌握率及护理满意率显著高于对照组(P<0.05),并发症发生率显著低于对照组(P<0.05).结论KTH整合式护理可以显著提高中晚期食管癌经外周静脉穿刺中心静脉置管化疗患者的自我效能感、自护能力及生活质量,帮助其掌握导管管理知识,降低并发症发生率,提高护理满意度.
Objective To observe the effect of KTH integrated nursing in PICC catheterization chemotherapy for patients with advanced esophageal cancer.Methods 120 patients with advanced esophageal cancer were divided into two groups according to the length of hospital stay,60 cases each.Both groups were given routine nursing intervention,and the observation group was given KTH integrated nursing intervention.Before and after the intervention,GSES was used to assess the self-efficacy of the two groups.ESCA assessed the self-protection ability of the two groups,EORTC QLQ-C30 assessed the quaitty of life of the two groups.Using the self-made catheter management knowledge mastery assessment questionnaire to assess the knowledge of two groups of catheter management knowledge.The incidence of complications was compared between the two groups.The self-care satisfaction assessment questionnaire was used to assess the satisfaction of the two groups.Results The GSES and ESCA scores of the two groups were significantly higher than those before the intervention(P<0.01).The observation group was more significantly increased than the control group(P<0.01).After the intervention,the emotional,cognitive,social,and overall status scores of the observation group of EORTC QLQ-C30 were significantly higher than those before the intervention(P<0.01).The cognitive scores of the control group were significantly higher than those before the intervention(P<0.01).The scores of the observation group were significantly higher than those of the control group(P<0.05 or 0.01).The knowledge of catheter management knowledge and nursing satisfaction rate in the observation group was significantly higher than that in the control group(P<0.05),and the complication rate was significantly lower than that in the control group(P< 0.05).Conclusions KTH integrated nursing can significantly improve the self-efficacy,self-care ability and quality of life of patients with advanced esophageal cancer in PICC catheterization chemotherapy,help them master catheter managem
