摘要
目的 探讨二尖瓣机械瓣置换同步射频消融术对风湿性心脏病合并心房颤动患者术后左心房内径及窦性心律转复维持率的影响.方法 将113例风湿性心脏病合并心房颤动患者按治疗方案分为研究组(57例)、对照组(56例).对照组行二尖瓣机械瓣置换术治疗,研究组行二尖瓣机械瓣置换术同步射频消融术治疗,术后随访6个月.比较两组术前及术后6个月左心房内径,随访6个月统计窦性心律转复维持率.结果 术后6个月两组左心房内径均较术前显著缩小(P<0.01),研究组显著小于对照组(P<0.01).术后6个月研究组窦性心律转复维持率为75.4%,对照组为44.6%,研究组显著高于对照组(P<0.01).结论 二尖瓣机械瓣置换同步射频消融术治疗风湿性心脏病合并心房颤动能有效恢复左心房内径,提高心房纤颤转复率,疗效稳定.
Objective To explore the effect of simultaneous radiofrequency ablation with mechanical mitral.valve replacement on postoperative left atrial diameter and maintenance rate of sinus cardioversion in pa-tients with rheumatie heart disease and atrial fibrillation.Methods 113 patients with rheumatic heart dis-ease complicated with atrial fibrillation were selected and divided into study group(n=57)and control group(n=56)according to the treatment plan.Mitral valve replacement was performed in the control group and synchronous radiofrequency catheter ablation was performed in the study group.Patients were followed up for 6 months.The left atrial diameter before and after operation was compared between the two groups.and the maintenance rate of sinus rhythm was recorded after 6 months follow-up.Results The left atrial diameter of both groups reduced 6 months after operation(P<0.01),and that in the study group was reduced than that in the control group(P<0.01).The maintenance rate of sinus rhythm was 75.4%in the experimental group at 6 months after surgery,which was higher than that of the control group 44.6%(P<0.01).Conclusions Mitral mechanical valve replacement combined with radiofrequen-cy ablation for rheumatic heart disease with atrial fibrillation can effectively restore the left atrial diameter.increase the conversion rate of atrial fibrillation,and have a stable effect.
