摘要
目的探讨产前超声检查在胎儿泌尿系统畸形诊断中的应用及临床价值。方法以2015年7月-2018年7月该院收治的4 600例孕妇作对象,施予产前超声检查,记录泌尿系统畸形诊断结果,并与产后随访结果进行比较。结果产后确诊有70例泌尿系统畸形胎儿,发生率1.52%。产前超声检查发现73例,出现2例漏诊及5例误诊,产前超声技术符合率99.85%(4 593/4 600),特异度99.89%(4 526/4 531),灵敏度97.10%(67/69),阴性预测值99.96%(4 526/4 528),阳性预测值93.06%(67/72)。结论产前超声检查在胎儿泌尿系统畸形诊断中的应用可获得较高准确率,可作为胎儿泌尿系统畸形预后评估及处理的重要依据,而且对于提升胎儿泌尿系统畸形检出率、降低其出生率具有积极意义。
Objective To explore the application and clinical value of prenatal ultrasonography in diagnosis of fetal urinary system malformations. Methods From July 2015 to July 2018,4 600 pregnant women treated in the hospital were selected,prenatal ultrasonography is performed,the diagnostic results of fetal urinary system malformations were recorded,then the results were compared with postpartum followup results. Results Seventy fetuses were diagnosed as urinary system malformations after birth,the incidence rate was 1. 52%. Seventythree fetuses were diagnosed as urinary system malformations before birth,two fetuses were missed diagnosed,and five fetuses were misdiagnosed. The coincidence rate,specificity,sensitivity,negative predictive value,and positive predictive value of prenatal ultrasonography were99. 85%( 4 593/4 600),99. 89%( 4 526/4 531),97. 10%( 67/69),99. 96%( 4 526/4 528),and 93. 06%( 67/72),respectively.Conclusion The accuracy rate of prenatal ultrasonography in diagnosis of fetal urinary system malformations is high,prenatal ultrasonography can be used as one of the important basis for prognosis evaluation and treatment of fetal urinary system malformations,which is of active significance in improving the detection rate of fetal urinary system malformations and reducing the birth rate of neonates with urinary system malformations.
