孕早期超声检测颈部透明层增厚与静脉导管频谱异常在筛查胎儿复杂先天性心脏畸形中的价值 被引量:11

The value of early pregnancy ultrasound detection of cervical translucency thickening and venous catheter spectrum abnormalities in screening fetal complex congenital heart malformations

摘要 目的研究采用静脉导管频谱(DV)和颈部透明层(NT)检测对孕早期检测胎儿复杂先天性心脏畸形的意义,分析其诊断价值。方法选取2018年1月至2019年1月来我院产检的3628例孕妇作为研究对象。所有研究对象均为孕早期孕妇,并在孕早期时给予所有孕妇NT和静脉导管多普勒血流频谱检测,分析DV和NT对胎儿复杂先天性性脏病的诊断价值。结果3628例孕妇中健康正常胎儿3606例,复杂先天性心脏畸形胎儿22例。在NT增厚、NT正常方面两组对比差异有统计学意义(P<0.05);在DV异常、DV正常方面两组对比差异有统计学意义(P<0.05);在NT增厚且DV异常、NT增厚或DV异常方面两组对比差异有统计学意义(P<0.05)。NT、DV筛查复杂先天性心脏畸形胎儿的灵敏度、特异度以及准确度均较高。NT、DV与复杂先天性心脏畸形呈明显正相关,均P<0.05。结论NT、DV对孕早期胎儿复杂先天性心脏畸形具有良好诊断价值,且NT、DV与胎儿复杂先天性心脏畸形的发生率呈正相关,为临床诊断胎儿先天性复杂心脏畸形起到重要价值。 Objective:To study the significance of venous catheter spectrum(DV)and cervical translucency(NT)in the detection of fetal complex congenital heart malformation in early pregnancy,and analyze its diagnostic value.Methods:248 pregnant women who came to our hospital from January 2018 to January 2019 were selected as the research objects.All the subjects were pregnant women in early pregnancy,and all pregnant women were given NT and venous catheter Doppler blood flow spectrum detection in the early pregnancy,the diagnostic value of DV and NT for fetal complex congenital heart disease were analyzed.Results:Among 248 pregnant women,216 cases were normal and 32 cases were complicated congenital heart malformation.There was significant difference in NT thickening and normal NT between the two groups(P<0.05);There was significant difference in DV abnormality and DV normal between the two groups(P<0.05);There was significant difference in NT thickening and DV abnormality,NT thickening or DV abnormality between the two groups(P<0.05).The sensitivity,specificity and accuracy of NT and DV screening for complex congenital heart malformations were high.NT and DV were positively correlated with complex congenital heart malformation(P<0.05).Conclusion:NT and DV have good diagnostic value for complex congenital heart malformation of fetus in early pregnancy,and NT and DV have positive correlation with the incidence of complex congenital heart malformation,which play an important role in clinical diagnosis of fetal complex congenital heart malformation.
