摘要
目的探讨超声造影联合CT合磁共振成像(MRI)在肝癌诊断中的应用价值。方法选取2018年3月至2020年8月间福建省漳浦县医院收治的72例肝癌患者纳入肝癌组,同期收治的72例肝良性肿瘤患者纳入良性组。所有患者都给予超声造影联合CT/MRI成像,记录影像学特征并判断诊断价值。结果肝癌组的显影时间(AT)与造影剂平均通过时间(MTT)均高于良性组,差异均有统计学意义(均P<0.05)。两组患者的达峰时间(TTP)与峰值强度(PI)值比较,差异无统计学意义(P>0.05)。肝癌组的肝动脉灌注量(HAP)、总肝灌注量(TLP)和肝动脉灌注指数(HPI)均高于良性组,差异均有统计学意义(均P<0.05)。两组患者的门静脉灌注量(PVP)值比较,差异无统计学意义(P>0.05)。肝癌组的肝病灶-肝实质的对比噪声比(CNR)高于良性组,差异有统计学意义(P<0.05)。超声造影联合CT和MRI诊断的敏感性与特异性为95.8%和98.6%。结论超声造影联合CT和MRI能很好地反映肝癌的影像学特征与血流特征,在肝癌的鉴别诊断中具有较高的敏感性与特异性。
Objective To explore the application values of contrast-enhanced ultrasound combined with computed tomography(CT)and magnetic resonance imaging(MRI)imaging for the diagnosis of liver cancer.Methods From March 2018 to August 2020,72 patients with liver cancer treated at Zhangpu County Hospital were selected and included in the liver cancer group.Another 72 patients with benign liver tumors were included in the benign group.Contrast-enhanced ultrasound combined with CT/MRI imaging were performed on all patients.Imaging characteristics were recorded and the diagnostic value was analyzed.Results Arrival time(AT)and mean transit time(MTT)were higher in the liver cancer group than in the benign group(all P<0.05).There was no significant difference in time to peak(TTP)and peakintensity(PI)between the two groups(P>0.05).Hepatic arteial perfusion(HAP),total liver perfusion(TLP)and hepatic perfusion index(HPI)were higher in the liver cancer group than in the benign group(all P<0.05).There was no significant difference in portal vein perfusion(PVP)between the two groups(P>0.05).Contrast-to-noise ratio(CNR)for liver lesion-liver parenchyma was higher in the liver cancer group than in the benign group(all P<0.05).The sensitivity and specificity of contrastenhanced ultrasound combined with CT and MRI were 95.8%and 98.6%,respectively.Conclusion Contrast-enhanced ultrasound combined with CT and MRI imaging can well reflect the imaging characteristics and blood flow characteristics of liver cancer with high sensitivity and specificity for the differential diagnosis of liver cancer.
