摘要
目的分析不同剂量氟伐他汀治疗老年冠心病的效果及对患者肿瘤坏死因子α(TNF-α)、单核细胞趋化蛋白-1(MCP-1)、白细胞介素18(IL-18)及血液流变学的影响。方法选取我院2018年1月至2019年12月收治的100例老年冠心病患者作为研究对象,采用随机数字表法将其分为对照组和观察组,各50例。两组均采用氟伐他汀治疗,对照组40 mg/d,观察组80 mg/d。比较两组的临床疗效、炎症因子水平及血液流变学指标。结果观察组治疗总有效率高于对照组,但差异无统计学意义(P>0.05)。治疗后,两组TNF-α、MCP-1、IL-18水平均低于治疗前,且观察组低于对照组(P<0.05)。治疗后,两组血浆粘度、纤维蛋白、红细胞压积均低于治疗前,且观察组低于对照组(P<0.05)。结论相较于氟伐他汀40 mg/d,80 mg/d更能改善老年冠心病患者的临床症状和体征,且有助于抑制炎性因子的过度表达,改善患者血液流变学。
Objective To analyze the effect of different doses of fluvastatin in the treatment of elderly patients with coronary heart disease and its influence on tumor necrosis factor-α(TNF-α),monocyte chemoattractant protein-1(MCP-1),interleukin-18(IL-18)and hemorheology.Methods A total of 100 elderly patients with coronary heart disease in our hospital from January 2018 to December 2019 were selected as the research objects,and the patients were divided into control group and observation group by random number table method,with 50 cases in each group.Both groups were treated with fluvastatin,40 mg/d in the control group and 80 mg/d in the observation group.The clinical efficacy,inflammatory factor levels and hemorheology indexes of the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group,but the difference was not statistically significant(P>0.05).After treatment,the levels of TNF-α,MCP-1 and IL-18 of the two groups were lower than those before treatment,and those of the observation group were lower than the control group(P<0.05).After treatment,the plasma viscosity,fibrin and hematocrit of the two groups were lower than those before treatment,and those of the observation group was lower than the control group(P<0.05).Conclusion Compared with fluvastatin 40 mg/d,80 mg/d can improve the clinical symptoms and signs of elderly patients with coronary heart disease,and it is helpful to inhibit the overexpression of inflammatory factors and improve the hemorheology of patients.
