摘要
目的探讨2型糖尿病(T2DM)女性患者绝经后骨密度与血清25羟维生素D[25(OH)D]水平的关系。方法检测276例绝经后T2DM女性患者的腰椎、股骨颈和全髋骨密度,据此将其分为骨量正常组(88例)、骨量减少组(98例)和骨质疏松组(90例)。同时也测定了HbA1c、血钙、血磷、ALP和25(OH)D水平。分析骨密度与25(OH)D水平的关系。结果276例绝经后T2DM女性患者中,维生素D充足者23例(8.33%),不足者47例(17.03%),缺乏者118例(42.75%),严重缺乏者88例(31.88%);91.67%绝经后T2DM女性患者存在25(OH)D相对或绝对缺乏。骨质疏松组和骨量减少组25(OH)D水平低于骨量正常组(P<0.05)。Logistic回归分析显示,年龄、BMI和25(OH)D水平可能为绝经后T2DM女性患者发生骨质疏松的独立影响因素(P<0.01)。结论绝经后的T2DM女性患者普遍存在维生素D缺乏,骨质疏松者尤为明显,需要适当补充维生素D。
Objective To investigate the relationship of bone mineral density and serum 25 hydroxy vitamin D[25(OH)D]level in postmenopausal women with type 2 diabetes mellitus(T2 DM).Methods The bone mineral density of the lumbar spine,femoral neck and total hip was detected in 276 postmenopausal women with T2 DM,who were assigned into three groups of A(with normal bone mass,88 cases),B(with osteopenia,98 cases)and C(with osteoporosis,90 cases).Blood levels of HbA1 c,calcium,phosphorus,ALP and 25(OH)D were determined as well.The relationship of bone mineral density and serum 25(OH)D level was analyzed.Results Of 276 postmenopausal women with T2 DM,23 cases(8.33%)were with vitamin D adequacy,47 cases(17.03%)were with vitamin D inadequacy,118 cases(42.75%)were with vitamin D deficiency,and 88 cases(31.88%)were with severe vitamin D deficiency.Relative or absolute vitamin D deficiency existed in 91.67%of postmenopausal women with T2 DM.Blood level of 25(OH)D was lower in groups of B and C than that in group A(P<0.05).Logistic regression analysis showed that age,BMI and 25(OH)D level may be the independent factors for osteoporosis in postmenopausal women with T2 DM(P<0.01).Conclusion The vitamin D deficiency is common in postmenopausal women with T2 DM,especially in those with osteoporosis,who need appropriate vitamin supplements.
