硫辛酸对早期糖尿病肾病患者体内氧化应激、炎性反应的调节及足细胞的保护作用 被引量:4

摘要 目的观察硫辛酸对早期糖尿病肾病(DKD)患者体内氧化应激、炎性反应的调节作用及足细胞的保护作用。方法选择2016年1月-2017年12月就诊于安徽省太湖县人民医院内一科的早期DKD患者60例,采用随机数字表法分为治疗组和对照组各30例。治疗组给予硫辛酸注射液静脉滴注,对照组给予0.9%氯化钠注射液静脉滴注。比较2组患者治疗前后空腹血糖、总胆固醇、三酰甘油、尿素氮、血清肌酐、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、超氧化物歧化酶(SOD)、丙二醛(MDA)、24 h尿蛋白排泄率及尿液足细胞标志蛋白(PCX)。结果治疗后,2组患者空腹血糖、三酰甘油及总胆固醇水平均较治疗前下降,且治疗组三酰甘油、总胆固醇水平均低于对照组(P<0.05),但2组空腹血糖水平比较差异无统计学意义(P>0.05)。治疗后,2组患者血肌酐、尿素氮水平均低于治疗前,且治疗组血肌酐低于对照组(P<0.05),但2组尿素氮水平比较差异无统计学意义(P>0.05)。治疗后,2组患者hs-CRP及IL-6水平均低于治疗前,且治疗组低于对照组(P<0.01)。治疗后,2组患者SOD水平均升高,MDA水平均下降,且治疗组变化幅度大于对照组(P<0.01)。治疗后,2组患者24 h尿蛋白排泄量及PCX水平均低于治疗前,且治疗组低于对照组(P<0.01)。结论硫辛酸可有效抑制早期DKD患者体内氧化应激及炎性反应水平,减轻足细胞损伤,保护肾功能。 Objective To observe the regulation of oxidative stress and inflammatory response in the body and podocyte protection of lipoic acid in patients with early diabetic kidney disease(DKD).Methods From January 2016 to December 2017,60 patients with early DKD,who were treated in the Internal Medicine of Taihu County People’s Hospital,Anhui Province,were selected and divided into treatment group and control group with 30 cases each by random number table.The treatment group was given intravenous drip of lipoic acid injection,and the control group was given intravenous drip of 0.9%sodium chloride injection.Compared the fasting blood glucose,total cholesterol,triacylglycerol,urea nitrogen,serum creatinine,high sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),superoxide dismutase(SOD),malondialdehyde(MDA),24-hour urine protein excretion rate and urine podocyte marker protein(PCX)between the two groups before and after treatment.Results After treatment,the fasting blood glucose,triacylglycerol and total cholesterol levels of the two groups were lower than before treatment,and the triacylglycerol and total cholesterol levels of the treatment group were lower than those of the control group(P<0.05).However,there was no significant difference in fasting blood glucose levels between the two groups(P>0.05).After treatment,the blood creatinine and urea nitrogen levels of the two groups were lower than before treatment,and the blood creatinine of the treatment group was lower than that of the control group(P<0.05),but there was no significant difference in the levels of urea nitrogen between the two groups(P>0.05).After treatment,the hs-CRP and IL-6 levels of patients in the two groups were lower than before treatment,and the treatment group was lower than the control group(P<0.01).After treatment,the SOD level of the two groups increased,and the MDA level decreased,and the treatment group changed more than the control group(P<0.01).After treatment,the 24-hour urine protein excretion and PCX levels of the two groups w
