摘要
目的:总结全国基层名老中医传承工作室指导老师赵华英采用中医综合疗法治疗乳痈的临床经验。方法:对近2年来跟师诊治的乳痈患者400余例,采用分期辨证论治,中医综合疗法治疗。初期:中药口服(荆防牛蒡汤加减)+抓揉排乳法+金黄膏(《医宗金鉴》)外敷。毒热期:中药口服(清热解毒、理气消肿为主)+金黄膏外敷,必要时口服或静点抗生素。热退后,抓揉排乳法(手法需轻柔)+金黄膏外敷+针刺(穴位+围刺)。脓肿期:火针烙口+手法排脓+中金黄膏外敷+中药口服(瓜蒌牛蒡汤合透脓散加减)+针刺。溃后期:中药口服(托里消毒散加减)+手法排脓+金黄膏外敷。脓量很少后加针灸(穴位+围刺)+微波/艾灸。整理诊疗方案,分析中药口服及外用法的疗效。结果:采用中医综合疗法治疗的400余例患者,疗效好,总治愈率达95%。结论:赵华英采用中医综合疗法治疗乳痈的方案,辨证分期论治,见效快、病程短、痛苦小,值得临床推广应用。
Objective:To summarize the clinical experience of Zhao Huaying,the instructor of the national grassroots famous old Chinese medicine inheritance studio,in the treatment of mammary carbuncle with TCM comprehensive therapy.Methods:In the past two years,more than 400 cases of mammary carbuncle patients treated by doctors were treated by stages and syndrome differentiation and TCM comprehensive therapy.Initial stage:oral administration of traditional Chinese medicine[Modified Jingfang Niubang decoction(荆防牛蒡汤)]+grasping and rubbing method+Jinhuang paste(金黄膏)(Yizong Jinjian)external application.Toxic heat period:oral Chinese medicine(clearing away heat and detoxification,regulating qi and swelling)+Jinhuang paste external application,oral or static antibiotics when necessary.After heat receding,apply grasping and kneading the milk drainage method(the technique should be gentle)+Jinhuang paste applied externally+acupuncture(acupoints+peri-acupuncture).Stage of abscess:burning with fire needle+manipulation of pus discharge+external application of Jinhuang paste+oral Chinese medicine[Guolou Niubang decoction(瓜蒌牛蒡汤)combined with Modified Tounong powder(透脓散)]+acupuncture.Later stage of ulcer:oral administration of Traditional Chinese medicine[Modified Tuoli Xiaodu powder(托里消毒散)]+manipulation of purulent+Jinhuang paste external application.Add acupuncture(acupoint+perithorn)+microwave/moxibustion after the amount of pus was very small.The treatment scheme was sorted out,and the efficacy of oral and external use of Chinese medicine was analyzed.Results:More than 400 patients were treated with TCM comprehensive therapy with good curative effect and total cure rate of 95%.Conclusion:Zhao Huaying's plan of applying the traditional Chinese medicine comprehensive therapy to treat the breast carbuncle has the advantage of dialectical stage treatment,quick effect,the disease course is short,the pain is small,and it is worthy of clinical popularization and application.
