摘要
目的探讨血清人β-防御素2(HBD2)在肠道病毒71型(EV71)手足口病中的表达及意义。方法收集2018年4月至2019年6月在西安市儿童医院感染科住院的EV71型手足口病患儿90例为病例组,其中包括普通型60例(普通型组)和重型30例(重型组),收集同期内健康体检的儿童50例做为正常对照组,用ELASA方法检测各组外周血清HBD2的水平,流式细胞仪检测各组外周血CD3^(+)、CD4^(+)、CD8^(+)T淋巴细胞、自然杀伤细胞(NK)、B淋巴细胞百分比含量,速率散射比浊法检测各组外周血免疫球蛋白IgG、IgM和IgA的浓度,对结果分别进行组间比较。结果EV71型手足口病血清HBD2水平高于正常对照组,并随病情加重而增高,差异有统计学意义(P<0.05);CD3^(+)、CD4^(+)、CD8^(+)T淋巴细胞、NK细胞水平均低于正常对照组,并随病情加重而逐渐下降,差异有统计学意义(P均<0.05);各组B淋巴细胞差异无统计学意义(P>0.05);手足口病患儿血清HBD2水平与其IgM水平呈一定程度的负相关(r=-0.52,P<0.05)。结论HBD2做为固有免疫效应分子在EV71感染后被诱导产生,随病情加重而升高,在手足口病适应性免疫功能抑制的状态下,对维持免疫平衡等方面可能发挥重要作用。
Objective To investigate the expression and immune function of human beta-defensin 2(HBD2)in enterovirus71(EV71)-assoicated hand-foot-and-mouth disease(HFMD).Methods Total of 90 patients with EV71-associated HFMDwere collected in the hospital from April 2018 to June 2019,and were divided into the mild group(60 cases)and severegroup(30 cases).While 50 healthy children who received checkup during the same period were recruited in the controlgroup.The serum levels of HBD2 were detected by ELISA.The lymphocyte subsets in peripheral blood,including CD3^(+),CD4^(+),CD8^(+)T lymphocytes,natural killer cell(NK)and B lymphocytes were detected by flowcytometry in terms of countand percentage in total lymphocytes.And the serum levels of IgG,IgM and IgA were detected by rate nephelometry.Theresults were compared and statistically analyzed between these groups respectively.Results The serum levels of HBD2 inHFMD were significantly higher than control group(P<0.05),and increased with the aggravation of the disease.Thepercentages of such lymphocyte subsets as CD3^(+),CD4^(+),CD8^(+)T lymphocytes and NK cells were decreased sequentially inthe control group,the mild group and the severe group,which had a significantly difference in different groups(P<0.05).But no significant difference of B lymphocytes was observed among these groups(P>0.05).In addition,the level of HBD2 was negatively correlated with Ig M(r=-0.52,P<0.05).Conclusion HBD2 could be induced as an innate immuneeffector molecule by the infection of EV71;the serum level of HBD2 increased with the aggravation of the disease,andmight play an important role in maintaining immune balance under the state of adaptive immunosuppression of hand-foot-and-mouth disease.
