摘要
目的探讨肝叶(段)切除结合T管引流术对肝内胆管结石患者疗效的影响。方法以本院为单位,在2017年1月—2019年1月选取肝内胆管结石患者70例,将其随机分为两组,每组35例,对照组行胆总管空肠Roux-en-Y吻合术,观察组行肝叶(段)切除结合T管引流术,对比两组手术指标及1年复发率情况。结果两组术中出血量、手术时间、住院时间、肛门首次排气时间比较,差异均无统计学意义(P> 0.05)。观察组结石清除率(97.14%)与对照组(80.00%)相比,显著偏高(P <0.05),而术后1年时的结石复发率(5.71%)较之对照组(25.71%),明显偏低(P <0.05)。结论针对肝内胆管结石患者,行肝叶(段)切除结合T管引流术治疗,结石清除率高,复发率低,临床应用价值突出。
Objective To investigate the effect of liver lobe(section)resection combined with T-tube drainage on patients with intrahepatic bile duct stones.Methods 70 patients with hepatolithiasis in our hospital from January 2017 to January 2019 were randomly divided into two groups,35 cases in each group.The control group was treated with Roux-en-Y choledochojejunostomy,and the observation group was treated with hepatic lobectomy combined with T-tube drainage.The operation indexes and 1-year recurrence rate of the two groups were compared.Results There were no significant differences in intraoperative blood loss,operation time,hospitalization time and the first time of anal exhaust between the two groups(P>0.05).The stone clearance rate of the observation group(97.14%)was significantly higher than that 80.00%of the control group(P<0.05),and the stone recurrence rate at 1 year after operation(5.71%)was significantly lower than 25.71%of the control group(P<0.05).Conclusion For patients with intrahepatic bile duct stones,lobectomy combined with T-tube drainage has high stone clearance rate and low recurrence rate,which has outstanding clinical value.
