摘要
目的探讨不同敷料更换方法对早产儿经外周静脉置入中心静脉导管(peripher-ally inserted central catheter,PICC)并发症的影响,为选择适合新生儿的敷料更换方法提供依据。方法将2016年9月—2018年10月PICC置管的203例早产儿随机分为对照组和试验组,对照组敷料更换按照《实用新生儿护理学》中导管置管处PICC敷料更换方法,进行敷料更换,试验组敷料更换方法在原敷料更换方法基础上进行改良,使用改良后更换敷料方法进行敷料更换,比较两种敷料更换方法对早产儿PICC并发症的影响。结果试验组穿刺口感染率(1.031%)、医用粘胶相关性皮肤损伤率(3.093%)、导管移位和脱出发生率(4.124%)均低于对照组,差异均有统计学意义(P<0.05)。结论改良后敷料更换方法比传统的敷料更换方法更安全、有效,改良后敷料更换方法能较好地降低导管相关并发症,值得推广。
Objective Explore the effects of different dressing replacement methods on PICC complications in premature infants.The purpose of this study is to provide a basis for the selection of dressing replacement methods suitable for newborns.Methods The 203 premature infants with catheterization from September 2016 to October 2018 were randomly divided into the control group and the experimental group.The dressings in the control group were changed according to the PICC dressing replacement method of catheterization in practical neonatal nursing.The dressing replacement method in the experimental group was improved on the basis of the original dressing replacement method,and the improved dressing replacement method was used to change the dressing.Compared the effects of two dressing replacement methods on the complications of PICC in premature infants.Results The incidences of puncture site infection(1.031%),skin injury related to medical adhesive(3.093%)and catheter displacement or prolapse(4.124%)in the experimental group were significantly lower than those in the control group(P<0.05).Conclusion The improved dressing replacement method is safer and more effective than the traditional dressing replacement method,and the improved dressing replacement method can better reduce catheter-related complications,which is worth popularizing.
